Could Strength Training Help Prevent Diabetes During Pregnancy?

A structured strength-training program undertaken during pregnancy was associated with a striking reduction in gestational diabetes among women with overweight or obesity, according to a randomized clinical trial published in Acta Obstetricia et Gynecologica Scandinavica. The study followed 209 pregnant participants from the early second trimester through late pregnancy or delivery and found that none of the participants who adhered to the prescribed resistance-training program developed gestational diabetes. In the control group, by comparison, the condition occurred in 7.3% of participants. The findings add to growing evidence that carefully supervised physical activity may influence pregnancy metabolism, not merely general fitness or maternal comfort.

Gestational diabetes develops when the body cannot produce enough insulin to compensate for the rising insulin resistance that normally occurs during pregnancy. As the placenta grows, hormonal signals help direct more glucose toward the fetus, but this adaptation can leave maternal blood sugar elevated. Women who begin pregnancy with overweight or obesity may already have reduced insulin sensitivity, placing additional strain on the pancreas. Strength training could counter some of these effects by increasing glucose uptake into skeletal muscle, improving insulin signaling, and preserving or expanding metabolically active muscle tissue. These mechanisms provide a biological explanation for why resistance exercise might affect gestational diabetes risk, although the trial itself was designed to test outcomes rather than establish a single physiological pathway.

Participants assigned to the intervention began the strength-training program between gestational weeks 12 and 14 and continued until week 36 or delivery. The timing is significant because insulin resistance generally increases as pregnancy progresses, particularly during the second and third trimesters. The program was structured rather than based solely on general advice to remain active, and the study’s principal comparison focused on participants who adhered to the training plan versus those in the control group. This distinction is important: the reported zero rate of gestational diabetes applied to women who followed the program, not necessarily to every participant originally assigned to the strength-training group. Adherence often determines whether an exercise intervention produces measurable health effects in real-world settings.

The difference in gestational diabetes rates was not the only notable result. No cases of large birth weight were recorded among participants who adhered to the strength-training program, whereas 6.3% of participants in the control group had babies classified in that category. Excessive fetal growth can accompany maternal hyperglycemia because increased maternal glucose crosses the placenta, prompting greater fetal insulin production. Fetal insulin acts as a growth-promoting hormone, potentially increasing fat deposition and overall birth weight. Preventing or reducing maternal glucose elevations may therefore help limit this pathway. However, the birth-weight finding should be interpreted cautiously because the trial involved a relatively small number of participants, and differences in categorical outcomes can be sensitive to small changes in case numbers.

The researchers also reported improvements in quality of life among participants who trained. Women in the strength-training group described pregnancy symptoms as less frequent and less disruptive, suggesting that the intervention may have benefits extending beyond glucose regulation. Pregnancy-related discomfort can affect mobility, sleep, mood, and the ability to work or maintain daily routines. Resistance exercise may help by supporting muscular strength, posture, joint stability, and functional capacity as body weight and biomechanical demands change. Physical activity can also influence psychological well-being through improved sleep, reduced stress, and greater confidence in managing bodily changes. The study therefore presents strength training not only as a possible preventive strategy for gestational diabetes, but also as a form of supportive prenatal care.

Safety outcomes were reassuring. The investigators observed no differences between groups in adverse events, including preterm birth. This matters because pregnant people are often advised to be physically active, yet concerns about miscarriage, premature delivery, or fetal harm can discourage exercise, particularly resistance exercise. The absence of a detected increase in adverse outcomes supports the possibility that a properly designed and monitored program can be compatible with a healthy pregnancy for women without contraindications. It does not mean that every exercise routine is appropriate for every pregnancy. Medical conditions, obstetric complications, symptoms, and individual fitness levels must be considered before training begins, and exercise intensity and technique may need to change as pregnancy advances.

The trial’s results are especially relevant because gestational diabetes is becoming an increasingly important public-health concern. The condition raises the risk of pregnancy complications, including excessive fetal growth, operative delivery, and hypertensive disorders, and it also signals a higher long-term risk of type 2 diabetes for the mother. Children exposed to gestational diabetes may face increased risks of obesity and metabolic disease later in life. Current prevention strategies commonly emphasize nutrition, weight management before pregnancy, and general physical activity. A structured strength-training program could offer a more specific and potentially accessible option, particularly for patients who may not enjoy or tolerate prolonged aerobic exercise. Resistance exercise can be adapted using body weight, elastic bands, machines, or free weights, allowing programs to match different abilities.

Even so, the findings should not be interpreted as proof that strength training eliminates gestational diabetes. The study included 209 women, and the dramatic difference between groups may partly reflect the limited sample size and the analysis of adherence. The supplied report does not establish how many participants were initially assigned to each group, how many completed the intervention, what exercises and loads were used, or how frequently sessions occurred. It also does not indicate whether participants’ diets, daily activity outside the program, or previous exercise habits differed in ways that could have influenced outcomes. Because the participants had overweight or obesity, the results may not apply directly to people at lower metabolic risk. Larger studies will be needed to determine which training patterns are most effective, whether benefits persist across different populations, and how strength training compares with aerobic or combined exercise.

The investigators, led by corresponding author Teresa E. Santa Cruz of Hospital Universitario Donostia in San Sebastián, Spain, said the findings suggest that adherence to a structured strength-training program during pregnancy may reduce gestational diabetes risk while improving maternal well-being. The next challenge is translating that signal into routine prenatal care. Clinical programs would need clear safety criteria, qualified supervision, progression guidelines, and practical methods for helping participants remain consistent. If future research confirms the results, strength training could become an important component of pregnancy health recommendations, alongside nutritional counseling and standard glucose screening. For now, the trial offers a compelling message: when appropriately prescribed and medically suitable, building strength during pregnancy may support both metabolic health and quality of life at a time when both are under unusual physiological pressure.
