A new Turkish validation study has confirmed that a psychological instrument designed to measure patients’ motivation for healing works reliably and accurately in a Turkish-speaking cancer population, offering clinicians and researchers in Türkiye a rigorously tested tool for understanding one of the most consequential — yet often overlooked — dimensions of cancer care: the patient’s own inner drive to heal. The study, published as an open-access research article in the journal BMC Nursing, was conducted by Nisa Yavuzer Bayrak of Nevsehir Haci Bektas Veli University and Mizgin Demir and Gülcan Bahcecioglu Turan of Fırat University, and its findings carry practical weight far beyond the statistical tables on which they rest.

The instrument in question is the Motivation for Healing Scale, a 25-item measure organized into five distinct dimensions that captures how strongly a patient feels compelled, oriented, and committed to the healing process. In psychometric terms, motivation for healing is a latent construct — an internal psychological state that cannot be observed directly and must instead be inferred from carefully worded questionnaire items whose responses correlate in theoretically meaningful ways. For such a construct to be useful in a new language and culture, the scale must be translated and then re-validated from the ground up, a process the research team carried out among cancer patients at the oncology clinic of a university hospital in eastern Türkiye.

Why does motivation matter so much in oncology? Decades of clinical observation suggest that patients who actively engage with their treatment — who attend appointments, adhere to medication regimens, adopt healthier behaviors, and maintain hope in the face of adversity — often navigate the cancer journey differently from those who withdraw or feel passively carried along by the medical system. Healing motivation sits at the intersection of psychological resilience, health behavior, and treatment engagement, and it can fluctuate with disease stage, symptom burden, social support, and the emotional climate of care. A validated scale allows clinicians to move beyond intuition and measure this motivational state systematically, potentially flagging patients whose motivation has eroded before that erosion translates into disengagement from care.

The research was designed as a methodological study, the specialized genre of clinical research devoted to evaluating measurement instruments rather than testing treatments. Data were collected through face-to-face interviews with 202 patients receiving cancer treatment at the oncology clinic, a sample size consistent with widely accepted standards for confirmatory psychometric analysis, which typically require several participants per scale item to stabilize estimates of the relationships among items. Face-to-face administration is itself a meaningful methodological choice in oncology populations, where fatigue, pain, and visual impairment can compromise self-administered questionnaires and where an interviewer can clarify items without leading the respondent.

Validation proceeded along two complementary axes: validity, meaning whether the scale measures what it claims to measure, and reliability, meaning whether it does so consistently. On the validity side, the researchers first assessed content validity, which evaluates whether each item is relevant, clear, and representative of the underlying construct as judged against the original scale’s intent. The results were emphatic. Item-level content validity scores ranged between 0.80 and 1.00 — comfortably above the conventional 0.78 threshold for individual items — while the scale-level content validity index reached 0.96, an exceptionally high value indicating near-consensus that the translated items faithfully preserve the meaning and relevance of the original instrument.

Construct validity was then examined through confirmatory factor analysis, a statistical technique that tests whether the pattern of correlations among the 25 items conforms to a pre-specified theoretical structure — in this case, the original five-dimension, 25-item architecture of the Motivation for Healing Scale. Rather than allowing the data to generate factors freely, as exploratory analysis does, confirmatory factor analysis imposes the hypothesized model and quantifies how well the observed covariance matrix among items fits that model. The analysis verified the original five-factor structure, and the resulting fit indices indicated good model fit, meaning the statistical discrepancies between the hypothesized structure and the actual response data were small enough to conclude that the Turkish version reproduces the conceptual architecture of the original scale.

Reliability results were equally robust. Internal consistency — the degree to which items within a scale or subscale measure the same underlying attribute — was assessed using both Cronbach’s alpha and McDonald’s omega, the latter increasingly preferred because it relies on fewer assumptions about whether all items contribute equally to the measured construct. Cronbach’s alpha coefficients for the five subdimensions ranged from 0.778 to 0.909, all within or above the widely used 0.70 benchmark for acceptable reliability, while the complete 25-item scale achieved an alpha of 0.918 and a McDonald’s omega of 0.911 — values that sit firmly in the excellent range and indicate that the Turkish items hang together as coherent measures of healing motivation.

Consistency over time was addressed through test–retest reliability analysis. A measurement instrument used in clinical settings must produce stable results when a patient’s actual state has not changed; if scores fluctuate wildly from one administration to the next, the scale is measuring noise rather than the construct. Repeated measurement analysis in this study indicated that the Turkish scale maintained consistency across time points, satisfying a critical requirement for any tool intended to track patients’ motivational status longitudinally — for example, across chemotherapy cycles or during the transition from active treatment to survivorship care.

The cultural adaptation process underlying these statistics deserves attention in its own right. Translating a health-related scale is not a matter of lexical substitution; idioms, emotional connotations, and health beliefs differ across languages, and an item that resonates naturally in English can sound clinical, awkward, or even distressing in Turkish. Although the published report summarizes the outcomes rather than the full procedural detail, the strong content validity indices testify to a careful forward-translation, back-translation, and expert-review cycle that preserved conceptual equivalence. Ethical oversight was equally explicit: the study received approval from the Fırat University Non-Interventional Research Ethics Committee under approval number 2025/06–32 and was conducted in accordance with the Declaration of Helsinki, with written informed consent obtained from all participants before enrollment.

The clinical implications of a validated Turkish Motivation for Healing Scale extend across the cancer care continuum. For nurses — the professional group at the front line of symptom management and patient education in oncology clinics — the scale offers a structured way to assess motivational status during routine encounters, converting a subjective impression into a quantifiable score that can be documented, communicated, and tracked. For physicians and psycho-oncologists, a low motivational score could serve as a trigger for deeper psychosocial assessment or referral to supportive care services. For researchers, the instrument opens the door to Turkish-language studies examining how healing motivation relates to treatment adherence, quality of life, symptom burden, and clinical outcomes, and to intervention trials designed to strengthen motivation in patients whose drive to heal has faltered.

The authors also emphasize the scale’s potential to support individualized care strategies. Modern oncology increasingly embraces personalized medicine at the molecular level, tailoring therapies to tumor genetics; motivational assessment represents a parallel form of personalization at the psychosocial level, tailoring communication, education, and support to each patient’s psychological readiness to engage in healing. A patient with high healing motivation may thrive with self-management resources and ambitious rehabilitation goals, while a patient with depleted motivation may first need emotional support, symptom relief, or family involvement before behavioral goals become realistic. The scale gives care teams an evidence-based starting point for those conversations.

There are, as with all validation studies, boundaries to the findings. The sample was drawn from a single oncology clinic in eastern Türkiye, and although 202 participants is adequate for confirmatory analysis, generalization to other regions, treatment settings, or cancer types will require further research. The published version is also an early-access article, citable under a permanent DOI and subject to final editorial formatting before the Version of Record replaces it. Nonetheless, the psychometric evidence assembled — high content validity, confirmed five-factor structure, good model fit, excellent internal consistency, and demonstrated temporal stability — constitutes a comprehensive and persuasive case that the Turkish adaptation performs on par with the standards expected of a clinical measurement instrument.

The study arrives at a moment when supportive and psychosocial oncology is gaining institutional recognition worldwide, and when instruments validated in local languages are increasingly seen as prerequisites for equitable, culturally attuned care. By demonstrating that healing motivation can be measured reliably in Turkish cancer patients, the research team has contributed a practical tool to that broader effort — one that transforms an invisible psychological resource into data that clinics can act upon, and that gives patients’ inner drive to heal the scientific attention it has long deserved.
