Systemic sclerosis, a rare connective tissue disease often described as an orphan condition, remains one of the most challenging disorders in rheumatology. A newly published narrative review in BMC Rheumatology brings together the current state of knowledge on its management, drawing on recent guidelines from international rheumatological, respiratory and cardiovascular societies. The disease is characterised by significant clinical and molecular heterogeneity, driving multi-organ fibrosis, vasculopathy and immune dysregulation, and it carries a high burden of both morbidity and mortality.

The review, authored by Medha Kanitkar, Voon H. Ong and Christopher P. Denton, was published on 1 October 2026 as an open-access article. Its stated aim is to provide an updated roadmap for managing what the authors call a hard condition, by applying the latest advances in precision-based medicine. Rather than presenting new experimental data, the article synthesises current best practice from guidelines, translational studies, clinical research and randomised controlled trials.

A central theme of the review is the role of emerging technologies in improving how patients are classified and treated. The authors explore how multi-omics approaches and artificial intelligence could contribute to patient stratification and to the design of more intelligent clinical trials. Such tools are presented as part of a broader movement towards precision medicine in the field, where matching the right treatment to the right patient at the right time is described as a key goal.

Cellular therapies receive particular attention as new treatment targets. The review describes these as groundbreaking approaches that hold significant potential to add to the existing armamentarium of tools available against systemic sclerosis. The article frames these therapies within the wider context of recent therapeutic progress, while noting that they remain an area of active development rather than established routine care.

Despite the strides highlighted, the authors emphasise that substantial unmet need persists. They point to gaps in understanding the pathogenic mechanisms of the disease, as well as the non-lethal burden it imposes on patients. Achieving true disease modification, targeted to the appropriate patient population at the appropriate time, is described as the ultimate objective that current efforts are working towards.

The review’s positioning of recent society guidelines as state-of-the-art updates rooted in evidence-based management underscores how the field has consolidated its approach to the disease.

For researchers, the review’s discussion of multi-omics and artificial intelligence signals where the authors see the next phase of progress emerging, particularly in trial design and patient selection. For patients and clinicians, the message is one of cautious optimism: therapeutic options and management strategies are advancing, but the authors are explicit that understanding of the underlying mechanisms and the full burden of disease still lags behind what is needed for genuinely transformative care.

The article is available open access, and the published version notes that the early-release text is citable and carries a permanent DOI, subject to further editorial refinement before the final version of record.
