A community-based screening study in China has found that a combined measure of metabolic health and physical frailty is associated with the presence of liver fibrosis, with the link appearing to follow a strictly linear dose-response pattern.

The research, published in BMC Gastroenterology, drew on the Lishui Community Liver Health Cohort and focused on the glucose-triglyceride frailty index, or TyGFI, a composite measure that brings together markers of glucose and triglyceride metabolism with indicators of frailty.

In the cross-sectional study, researchers stratified participants by TyGFI tertiles and assessed liver fibrosis using vibration-controlled transient elastography. Binary logistic regression was then used to evaluate the association between the index and the presence of liver fibrosis.

After adjusting for potential confounders, participants in the highest TyGFI tertile showed a significantly increased risk of liver fibrosis compared with those in the lowest tertile, with an adjusted odds ratio of 3.28 and a 95 percent confidence interval of 2.04 to 5.30, a result that was statistically significant.

Restricted cubic spline analysis confirmed a strictly linear dose-response relationship between the index and liver fibrosis, with a P value for non-linearity of 0.241. The association remained consistent across all prespecified subgroups, with no statistically significant interaction effects.

The team also carried out an exploratory mediation analysis, which suggested that gamma-glutamyl transferase (GGT) partially mediated the association. According to the analysis, GGT accounted for 23.43 percent of the total effect, a finding the authors reported as statistically significant.

Sex-stratified analysis added a further layer of nuance. The potential GGT-mediated statistical association was prominent in males, with a P value of 0.014, whereas the mediation proportion was not statistically significant in females, where the P value was 0.70.

The authors concluded that, in a community-based screening setting, the TyGFI demonstrates a robust linear dose-response association with the presence of liver fibrosis, and that the exploratory GGT-mediated statistical link accounts for nearly a quarter of the association, with pronounced male susceptibility.

Importantly, the researchers framed their conclusions cautiously. They noted that assessing these factors may aid in the sex-tailored identification of individuals with liver fibrosis, but suggested that using the additive TyG plus frailty index model, or the TyG index alone, may be more practical for clinical implementation.

As a cross-sectional analysis of a screening population, the study identifies associations rather than proving causation, and the mediation findings are described by the authors as exploratory.
