A new study in BMC Medical Genomics points to the transcription factor FOXO1 as a candidate player in developmental dysplasia of the hip (DDH), a common pediatric and adult hip disorder that can lead to lasting hip dysfunction and whose pathogenesis remains incompletely understood. The researchers set out to clarify how FOXO1, a known regulator of cell proliferation and apoptosis, acts in DDH chondrocytes and to evaluate its potential clinical diagnostic and prognostic value.

The team began with a transcriptomic analysis of the GSE169495 dataset to identify dysregulated genes in DDH chondrocytes. FOXO1 and its regulatory network, including Akt1 and Gsk3b, were persistently dysregulated in the dysplastic cartilage cells. The authors then verified FOXO1 expression and function in human DDH chondrocytes in the laboratory, using siRNA knockdown, flow cytometry to measure cell cycle status and apoptosis, CCK-8 assays to track proliferation, and RT-PCR.

These functional assays indicated that FOXO1 restrains chondrocyte proliferation by driving cells into G0/G1 cell-cycle arrest, a shift accompanied by downregulation of PCNA, Cyclin D1 and p21, and that it promotes apoptosis. In other words, within this experimental system FOXO1 behaved as an inhibitor of chondrocyte growth and a promoter of programmed cell death, processes central to cartilage homeostasis.

To test clinical relevance, the researchers analyzed data from 120 DDH patients, divided into 40 mild, 40 moderate and 40 severe cases, alongside 60 healthy controls. Preoperative FOXO1 levels correlated negatively with DDH severity, meaning higher expression was associated with milder disease in this cohort. Within the cohort, FOXO1 also showed high diagnostic performance, with an area under the curve of 0.994 (95% CI 0.986–0.999), and it predicted postoperative improvement in Harris hip scores.

The authors conclude that FOXO1 acts as a candidate regulator of DDH pathogenesis and a promising clinical biomarker within the studied cohort, offering a potential target for DDH diagnosis and treatment. They emphasize that the findings warrant independent validation before any clinical application, and the study is published as an open-access article in BMC Medical Genomics on 14 October 2026.

Subject of Research: The role of the transcription factor FOXO1 in chondrocyte proliferation, apoptosis and clinical severity in developmental dysplasia of the hip.
