Congenital and genetic disorders often cause death or lifelong disability, and treatments delivered after birth are frequently unable to reverse the effects of disease that has already become established. A review published in Nature Reviews Bioengineering by Elizabeth Reynolds, Diana Farmer and Aijun Wang examines how bioengineering strategies could move intervention earlier, into the fetal environment itself, where existing fetal surgical therapies have already demonstrated that in utero treatment can address conditions during development.

The authors argue that the fetal environment is an ideal treatment setting for certain congenital and genetic diseases. According to the review, this advantage stems from the fetus’s relative immaturity, rapid proliferation and superior wound healing compared with the postnatal state. The review situates current efforts within a longer history, tracing major milestones in utero interventions and cataloguing the distinct advantages of the fetal environment as a treatment setting.

Three strategy families anchor the review: tissue engineering, stem cell transplantation and gene-based approaches. The authors present these as emerging bioengineering strategies for fetal treatments that have advanced the field and continue to provide new opportunities for unmet clinical needs, including conditions for which no treatment or only inadequate treatments currently exist. The review further discusses how these methods could overcome existing limitations and improve the translation of in utero therapies into clinical use.

Translating these concepts into safe clinical practice remains a central obstacle. The review emphasizes challenges related to the safe delivery of therapies and to the careful consideration of risks to both the mother and the fetus. In utero drug delivery mechanisms are among the topics the review examines as part of this discussion of delivery and precision.

The authors also stress that progress depends on multidisciplinary partnerships between clinicians, biologists, bioengineers, fetal centre staff and patients, which they identify as critical for the continued advancement of fetal medicine. As the field develops, the review positions bioengineering as a route to extend prenatal intervention for congenital and genetic disorders that today have few or no treatment options, while acknowledging that safety for mother and fetus must remain central to any clinical translation.
