CLEVELAND, Ohio (July 29, 2026)—A large new analysis of more than 107,000 women suggests that menopause timing is linked to later development of high blood pressure. The association appears strongest for women who reach menopause well before typical age. The study, published online in Menopause, reports that cardiovascular risk may be concentrated in a narrower window than the conventional definition of “premature” menopause.

Scientists have long known that hormonal shifts during the menopause transition can affect overall health. Early menopause has previously been tied to higher rates of coronary heart disease and stroke, but whether it directly increases hypertension risk has remained uncertain. Earlier meta-analyses have produced conflicting results, partly because the menopause transition often coincides with weight gain and metabolic changes that also raise blood pressure.

To reduce that problem, the researchers used data from the UK Biobank, focusing on 107,836 postmenopausal participants enrolled between 2006 and 2010. Women were followed for a median of nearly 15 years through the end of 2003. Instead of treating menopause as a single event, the team categorized participants by age at menopause—normal (after 45), early (40–45), and premature (before 40)—and also examined natural versus surgical menopause.

Across follow-up, 18,508 women (17.2%) were diagnosed with hypertension. Rates rose as menopause occurred earlier: 16.6% among women with normal timing, 18.8% among those with early menopause, and 22.6% among those with premature menopause. After statistical adjustment for more than 50 factors—including body weight, lifestyle, family history, and laboratory measures—premature menopause still corresponded to a higher risk of developing hypertension.

The investigators then modeled age at menopause on a continuous scale, finding that risk peaked between ages 25 and 35 rather than exactly at the traditional cutoff of 40. This suggests that the biologically relevant exposure window may begin earlier than standard clinical categories imply.

In supplementary analyses, surgical menopause initially showed higher hypertension rates, but the association weakened after accounting for other cardiovascular risk variables. Taken together, the results strengthen the case that the timing of menopause itself—rather than only associated behaviors—may help forecast long-term blood pressure outcomes.

The authors recommend that clinicians consider age at menopause as a distinct cardiovascular risk marker, particularly for women who experience menopause before age 40. They also emphasize earlier blood pressure screening and management as potential opportunities to reduce future cardiovascular burden. Hormone therapy is commonly recommended for premature menopause until the natural age of menopause, unless contraindications exist.
