Family socioeconomic status may matter more for children’s long-term health than specific prenatal habits like smoking, alcohol use, or caffeine intake, according to a new analysis published in PLOS Medicine. The study, led by Gemma Sharp at the University of Exeter, draws on data from multiple long-running parent–child cohorts in the United Kingdom and Norway.
Researchers evaluated whether prenatal and early-life exposures linked to parents could predict a wide range of child outcomes later on. Using the Developmental Origins of Health and Disease (DOHaD) framework, they tested how environmental influences might “program” health trajectories beginning in pregnancy and extending through childhood.
Unlike many prior studies that focus primarily on mothers, this work also incorporated fathers and the broader family context in which children are raised. Across more than 230,000 participants, the team ran several statistical models to estimate associations between parental health behaviors and 72 different child health outcomes measured at six time points.
Parental behaviors included smoking, drinking, and caffeine consumption, while exposures were also compared against the family’s socioeconomic position. Outcomes spanned physical measures and behavioral and mental health indicators, such as size at delivery, body mass, hyperactivity, social communication, aggression, and depressive symptoms.
The results showed that smoking, alcohol, and caffeine were linked to only a small fraction of the outcomes relative to socioeconomic status. Only 6% of results suggested associations for smoking, 3% for alcohol, and 0.4% for caffeine, whereas 15% of child health effects tracked with socioeconomic disadvantage.
Importantly, the researchers did not observe a consistent pattern suggesting mothers’ behaviors were more influential than fathers’. Instead, the association profiles for maternal and paternal smoking, alcohol, and caffeine resembled each other closely, hinting that prenatal effects may be intertwined with the wider family environment.
Although the study is observational—meaning it can’t prove causation—the authors argue that public health strategies could be more effective when they target structural socioeconomic inequalities rather than concentrating solely on individual parental choices during pregnancy.
“Our study suggests that the social and economic circumstances children grow up in may have a greater influence on their health than specific parental behaviours,” Sharp said, emphasizing the consistency of socioeconomic disadvantage across outcomes.
