ROCHESTER, Minn. — Pancreatic cysts are common, frequently discovered by chance during imaging performed for unrelated medical concerns, and usually do not become cancerous. Yet a small proportion can develop into pancreatic cancer or advanced precancerous lesions, creating a difficult clinical problem: physicians must identify patients who could benefit from surgery without exposing people at comparatively low risk to an invasive operation and its potential complications. In a prospective study published in Gastroenterology, Mayo Clinic researchers have now estimated how the number of “worrisome features” visible in a pancreatic cyst affects the likelihood that it will progress within three years. The results suggest that the risk is relatively modest when only one worrisome feature is present but rises sharply when several features occur together, offering physicians a more precise framework for surveillance and treatment decisions.
The study found that patients whose pancreatic cysts had one worrisome feature faced approximately a 2% risk of developing pancreatic cancer or advanced precancerous changes within three years. That estimate increased to about 4% among patients with two worrisome features and reached 18% among those with three or more. The findings indicate that the number of abnormalities may be as important as the presence of any single abnormality when doctors assess a cyst. Worrisome features can include changes detected on imaging that suggest biological activity or a greater chance of progression, while high-risk features are more strongly associated with malignancy and may lead clinicians to recommend surgery. The researchers’ analysis supports a more graduated approach, in which a patient with one concerning finding may be managed differently from someone whose cyst displays a cluster of high-risk characteristics.
“This information will help us counsel patients,” says Shounak Majumder, M.D., the study’s lead author and a gastroenterologist at Mayo Clinic. “If we want to intercept pancreatic cancer while also sparing patients from unnecessary interventions, we must really know what is likely to become cancer and what is not.” That balance is particularly important for pancreatic disease because surgery can be complex and carries meaningful risks, even when performed at experienced centers. At the same time, pancreatic cancer is often diagnosed late, when curative treatment is no longer possible. A cyst that appears harmless today may therefore generate prolonged uncertainty for patients and clinicians, especially when imaging cannot definitively reveal whether abnormal cells will remain stable, progress to a precancerous state or eventually become invasive cancer.
Researchers followed 230 people whose pancreatic cysts had worrisome or high-risk features. The prospective design is significant because participants were enrolled and monitored over time rather than identified only after their medical records were reviewed. Many earlier studies have been influenced by selection bias because they primarily included patients who underwent surgery, a group more likely to have had severe or suspicious disease. In the Mayo Clinic study, participants were followed regardless of whether they chose an operation or entered clinical surveillance. Thirty-two people underwent surgery within the first three months after enrollment, while 198 entered a surveillance program. Among those monitored, 185 had cysts with worrisome features and 13 had cysts classified as having high-risk features. This approach allowed investigators to examine how cysts behaved in patients who were not immediately treated surgically.
The results showed that multiple worrisome features identify a substantially higher-risk subgroup, whereas a single worrisome feature was not associated with a markedly elevated risk over the three-year period. That distinction could help doctors avoid treating every abnormal imaging finding as an automatic indication for surgery. Instead, clinicians may combine the number and type of features with cyst size, growth over time, involvement of the pancreatic duct, the patient’s age and health, symptoms, family history and surgical fitness. Pancreatic cysts without worrisome or high-risk characteristics generally have a relatively low probability of becoming malignant and are often observed with periodic imaging. Cysts with high-risk findings, by contrast, may prompt surgery because the likelihood of cancer or advanced precancerous change is considered sufficiently concerning. The new estimates provide a numerical context for discussions that have traditionally relied heavily on clinical judgment.
An additional observation from the study may be especially important for patients who undergo careful surveillance: cancers detected during monitoring were diagnosed at a very early stage. Although the study was not designed to prove that surveillance itself improves survival, the finding suggests that appropriately selected patients may have an opportunity for early intervention rather than facing an immediate operation based solely on uncertainty. Surveillance typically involves repeated imaging and, in selected cases, additional procedures or laboratory testing to look for changes in the cyst. The strategy requires reliable follow-up and careful risk reassessment, because a cyst’s appearance can change over time. The three-year progression estimates may also serve as a benchmark for evaluating new tests. If a blood test, imaging method or molecular assay is intended to identify cysts at greatest risk, researchers can compare its performance against the progression rates observed in this prospective cohort.
Mayo Clinic investigators also examined whether CA 19-9, a blood biomarker sometimes used in the evaluation or monitoring of pancreatic cysts, could improve risk prediction. CA 19-9 is a carbohydrate antigen that may be elevated in people with pancreatic cancer, but its concentration can also be influenced by noncancerous conditions, including obstruction or inflammation of the bile ducts. In a separate study led by Dr. Majumder and colleagues and published in Clinical Gastroenterology and Hepatology, an elevated CA 19-9 level by itself did not reliably identify which patients with pancreatic cysts would go on to develop cancer. Among patients with low-risk cysts who had elevated CA 19-9, none developed pancreatic cancer during the study’s follow-up period. The result cautions against interpreting the marker in isolation and highlights the limitations of relying on a single blood measurement to detect the earliest stages of pancreatic malignancy.
The researchers say CA 19-9 should be considered alongside imaging findings, clinical history and other measures of cancer risk rather than treated as a stand-alone decision tool. A normal result cannot guarantee that a cyst is harmless, and an elevated result does not necessarily mean that cancer is present. This uncertainty reflects the biology of pancreatic cancer, which can develop through several molecular pathways and may remain below the detection threshold of conventional tests during its earliest phases. New biomarkers could potentially analyze tumor-derived DNA, abnormal proteins, altered metabolites or molecular signals released into blood or cyst fluid. However, such tests must be validated in large, carefully followed populations to determine whether they can distinguish harmless cysts from lesions that are genuinely on a path toward invasion. The Mayo Clinic findings provide both a clinical reference point and a rationale for developing more sensitive and specific tools.
For patients, the central message is that a pancreatic cyst does not automatically mean pancreatic cancer, and immediate surgery is not always the safest or most effective response. The estimated three-year risk was about 2% with one worrisome feature, 4% with two and 18% with three or more, but individual risk can vary according to the precise features and the patient’s overall circumstances. Decisions should therefore be made through detailed discussions with specialists familiar with pancreatic disease, using high-quality imaging and a clear surveillance plan when observation is appropriate. Patients should also understand the importance of attending follow-up appointments, because changes in size, structure or symptoms may alter the recommended course. The researchers’ goal is not simply to reduce operations, but to make intervention more timely for those most likely to benefit while helping lower-risk patients avoid unnecessary procedures and the anxiety associated with uncertain diagnoses.
The two studies form part of a broader Mayo Clinic effort to improve early detection and prevention of pancreatic cancer through the Precure Research initiative. By establishing more precise estimates of progression risk, investigators can design larger studies and clinical trials that test whether biomarkers, advanced imaging and other technologies can identify dangerous disease before it becomes invasive. The long-term objective is personalized surveillance: a system in which monitoring intensity and treatment decisions reflect the biological risk of each cyst rather than a one-size-fits-all category. For now, the prospective data offer physicians a clearer way to explain risk and give patients more meaningful information when weighing surveillance against surgery. In a cancer known for late diagnosis and limited treatment windows, that improved precision could become an important step toward finding malignancy earlier while preserving quality of life for people whose cysts are unlikely to progress.
