Nearly half of university students across the six Gulf Cooperation Council countries are experiencing symptoms of depression, and more than half show signs of anxiety or stress, according to a sweeping new systematic review and meta-analysis published in BMC Psychiatry. The study, led by Ahmed Hossain of the University of Sharjah and North South University, pooled data from 46 studies covering 24,601 students across Saudi Arabia, the United Arab Emirates, Kuwait, Qatar, Bahrain and Oman, and found pooled prevalence estimates of 48.1 percent for depression, 53.3 percent for anxiety and 53.4 percent for stress. Perhaps most striking, the researchers project that these figures will continue climbing through 2030 if current trends persist, a forecast that the authors describe as a critical public health warning for universities and policymakers throughout the region.
The scale of the synthesis is itself noteworthy. The research team searched PubMed, Scopus, EBSCO, Web of Science and Google Scholar for studies published between January 2010 and March 2025, following PRISMA guidelines to identify, screen and include eligible research. The analysis was registered prospectively with PROSPERO, and because it relied solely on secondary analysis of previously published data, no new ethical approval was required. Across the four decades-spanning literature, depression estimates drew on 40 studies, anxiety on 34 and stress on 32, giving the pooled figures a breadth that individual campus surveys simply cannot match.
Methodologically, the team used random-effects meta-analysis to combine prevalence estimates, an approach that explicitly assumes the true prevalence varies across studies rather than assuming a single fixed value. Heterogeneity was assessed with the I-squared statistic, which quantifies how much of the variation between studies reflects genuine differences rather than chance. To probe that variation further, the researchers ran subgroup analyses stratified by country, sex, age group, field of study, whether data were collected before or during the COVID-19 pandemic, and the measurement instrument used. The projections to 2030 were generated using random-effects meta-regression on the logit scale, a technique that models the log-odds of prevalence over time and then back-transforms the fitted trend into expected prevalence figures.
The subgroup results sketch a detailed map of who is most at risk. Anxiety prevalence was higher among female students, at 54.4 percent, than among male students, at 46.7 percent. Stress was markedly elevated among medical students, reaching 60.2 percent compared with 49.9 percent among their non-medical peers, a gap consistent with the intense examination schedules, clinical workload and competitive culture associated with medical education. Depression prevalence jumped during the COVID-19 pandemic, rising to 57.8 percent from 45.2 percent in the pre-pandemic period, echoing global evidence that lockdowns, remote learning and social isolation amplified psychological distress in young adults. The authors also note that students in the early years of their degrees appeared among the vulnerable groups they highlight, alongside women and medical students.
Geography mattered as well. Oman and Qatar reported the highest anxiety and stress rates among the GCC countries included in the analysis, while the pooled estimates varied across nations, reflecting differences in study populations, timing and measurement tools as much as genuine country-level differences. The review relied heavily on validated screening instruments, including the Depression Anxiety Stress Scales-21, known as DASS-21, which is one of the most widely used self-report tools for quantifying symptom severity in student populations. Because such instruments measure symptoms rather than clinical diagnoses, the authors are careful to frame their findings as prevalence of symptoms, a distinction that matters for interpretation but does not diminish the public health signal.
The wide confidence intervals tell their own story. The pooled depression estimate of 48.1 percent carries a 95 percent interval of 43.0 to 53.8 percent, anxiety ranges from 48.0 to 59.0 percent, and stress spans 46.0 to 61.2 percent. These broad ranges reflect the substantial heterogeneity inherent in combining cross-sectional surveys conducted over fifteen years, across different countries, instruments and sampling frames. The researchers addressed this through sensitivity analyses and funnel plots examining publication bias, with full details provided in their supplementary materials, and they report prediction intervals alongside their pooled estimates to communicate the range in which a new study’s prevalence might realistically fall.
The projected upward trajectory to 2030 is arguably the study’s most consequential finding. Using meta-regression on the logit scale, the authors forecast that if the trends observed between 2010 and 2025 continue, prevalence rates for depression, anxiety and stress among GCC university students will keep rising over the next several years. This projection lands at a moment when the Gulf states are investing heavily in higher education, expanding enrollment and positioning their universities within international rankings. The analysis suggests that student wellbeing has not kept pace with that growth, and that without deliberate intervention, the psychological burden on the region’s student population will deepen.
The authors are direct about the implications. They describe the burden as alarmingly high and increasing, and they call for targeted interventions aimed at the highest-risk subgroups, particularly female students, students in early years of study and those in medical programs. They argue that universities and policymakers must prioritize and invest in robust mental health support systems, framing the projected rise to 2030 as a critical public health imperative tied to broader goals, including the Sustainable Development Goals and the work of the World Health Organization on mental health. In a region where stigma around psychological distress has historically limited help-seeking, the sheer visibility of these numbers may itself prove a catalyst for change.
Several caveats deserve attention. Most of the included studies were cross-sectional, meaning they capture symptoms at a single point in time and cannot establish causal relationships between risk factors and outcomes. The reliance on self-report screening tools means estimates depend on how students interpret and answer symptom questionnaires. Cultural factors specific to the Gulf, including the composition of student bodies, where international students form a substantial share in some countries, may shape both symptom reporting and prevalence in ways that are difficult to disentangle. The authors themselves registered the review prospectively and published their quality assessments using the JBI scale for cohort and cross-sectional studies, transparency that helps readers judge the strength of the evidence base.
Nevertheless, the study stands as the most comprehensive quantitative picture to date of student mental health in the Gulf Cooperation Council region. By synthesizing fifteen years of data and extending the trend line five years into the future, it converts scattered campus surveys into a coherent, actionable picture of a growing crisis. For the millions of students navigating increasingly competitive universities in one of the world’s fastest-changing regions, and for the administrators responsible for their welfare, the message from the data is unambiguous: mental health support in Gulf higher education is not a discretionary amenity but an urgent, measurable necessity, and the window for effective action is the present.
