Medical artificial intelligence is spreading through clinics worldwide, but a new international study reports that a structured set of bioethical guidelines for its everyday clinical use has now been agreed by a global panel of experts. The work, published in BMC Medical Ethics, describes a four-round eDelphi consensus exercise designed to establish best-practice guidance for the ethical use of medical AI in healthcare.

The study set out to address a gap identified by its authors: despite a growing body of ethical principles and emerging regulations, no standardized clinical bioethics framework existed to guide the practice of medical AI. The Delphi method was chosen to obtain global expert agreement on ethical challenges, principles, guidelines, and practical tools for the technology’s use in care settings.

Methodologically, the exercise was conducted under ACCORD and DELPHISTAR guidance and involved multidisciplinary stakeholders identified through systematic review and targeted recruitment. Preliminary survey items were derived from evidence and refined iteratively across rounds. Consensus was predefined as at least 80 percent agreement sustained across two consecutive rounds, and the surveys were anonymized and randomized. Ethical approval and protocol registration were secured before the study began.

The panel comprised 53 participants from 21 countries, with survey rounds running from November 2025 to February 2026. Of the 170 items considered, 132 reached the consensus threshold. The accepted items were organized into six categories: bioethical issues, values, principles, solutions, a checklist for clinicians, and report markers intended to support risk management.

According to the authors, the study produced clinician-focused practice guidelines for the bioethical use of medical AI, together with practical implementation tools intended to support institutional governance.

One notable negative finding: no new bioethical principles achieved consensus. The ethical components that did reach agreement aligned with prior literature, suggesting the panel consolidated existing ethical thinking rather than generating novel principles.

The authors acknowledge limitations, including geographic imbalance in the panel and uneven stakeholder representation.

Across the accepted items, the recommendations emphasized human responsibility, governance, and practical tools for implementation.

The authors conclude that the consensus provides practical, ethics-based guidance for the responsible integration of medical AI into clinical practice and institutional governance. As an early-access, peer-reviewed publication, the article is citable with a permanent DOI, though the version is subject to further edits before the final Version of Record replaces it.
