Low brain metabolism measured on ¹⁸F-FDG PET/CT may help forecast outcomes for people living with advanced non–small cell lung cancer (NSCLC), according to a new study published in The Journal of Nuclear Medicine. The work suggests that brain uptake, captured during routine PET/CT scans, can refine survival prediction beyond tumor and standard clinical factors.

While modern treatments have improved some patients’ trajectories, metastatic NSCLC remains difficult to manage. Even with promising therapies, overall survival spans a wide range—from years in early disease to only months once metastases emerge—highlighting the need for reliable, patient-specific prognostic signals.

“Metabolic activity and density across the body can reflect a patient’s overall condition,” said Julie Auriac, PhD, of IRIS in Orsay, France. The study aimed to test whether the brain’s ¹⁸F-FDG metabolic activity is linked to survival in advanced NSCLC, and whether combining it with existing biomarkers improves prognostic accuracy.

Researchers performed a retrospective analysis of advanced NSCLC patients who underwent pretreatment ¹⁸F-FDG PET/CT between 2010 and 2023. They collected clinical variables, tumor characteristics, blood biomarkers, and imaging-derived metrics, including total metabolic tumor volume (TMTV) and measures of brain metabolism (brain SUVmean).

Patients with higher brain metabolism showed better survival. Brain metabolism was significantly lower in individuals who died within one year compared with those still alive at that time point. Statistical models indicated brain metabolism was an independent prognostic factor for overall survival.

Crucially, brain metabolism was not merely redundant with other markers. It was significantly correlated—but complementary—to several clinical and blood biomarkers, suggesting that it adds distinct biological information about systemic disease burden or resilience.

The authors emphasize that the brain signal is obtained without extra procedures: it is available from PET/CT images already acquired during routine care. This makes brain metabolic activity a practical candidate for improving risk stratification early in the treatment pathway.
