Children who begin antiretroviral therapy within their first months of life appear to develop smaller, less diverse HIV reservoirs than adults, according to a review of pediatric reservoir biology. The review also identifies a distinctive immune profile in children, including greater thymic output, more naïve and central-memory CD4⁺ T cells, and less immune exhaustion. However, it stresses that the evidence is uneven: findings supporting restricted initial reservoir seeding are quantitatively stronger than evidence that reservoirs continue to decay during sustained viral suppression.

Antiretroviral therapy effectively suppresses HIV replication, but latent viral reservoirs remain the principal barrier to a cure. In infants receiving early ART, suppression coincides with a critical period of immune-system development. The pediatric reservoir therefore differs biologically from reservoirs established in adults.

Reviewing the available evidence, the authors describe smaller reservoirs and reduced viral diversity after early treatment, alongside a greater proportion of less-differentiated infected cells and lower expression of exhaustion markers. These characteristics may help explain why pediatric reservoirs behave differently from adult reservoirs. A smaller founding population and lower viral diversity could limit the cellular and viral variation available for clonal expansion and immune-driven viral evolution.

Reservoir measurements also depend on the assays selected and on the cellular or tissue compartments examined. The review identifies cellular and tissue compartmentalization as important determinants of how reservoir persistence is interpreted. Different assays can produce differing estimates of reservoir size, composition and persistence, so apparently inconsistent results require careful comparison of methods.

The authors emphasize that a clearer account of reservoir establishment, maintenance and dynamics is needed before age-appropriate latency-reversing, immunotherapeutic and gene-based interventions can be designed and evaluated. The relatively small and less diverse reservoirs observed after early treatment, together with better-preserved immune function, may provide a more favorable setting in which to test cure-oriented strategies. Progress will nevertheless depend on distinguishing the comparatively well-supported reduction in initial seeding from the less definitive, longitudinal and observational evidence for continued reservoir decay under sustained suppression.
