Tanzanian Study Links Hearing Loss to Hypertension

Sensorineural hearing loss may be a significantly overlooked complication of hypertension, according to a new study of adults receiving care at Kilimanjaro Christian Medical Centre (KCMC) in northern Tanzania. Researchers found that nearly one in four hypertensive patients assessed had measurable damage affecting the inner ear or auditory nerve, raising concerns that hearing health may be neglected in routine cardiovascular care.

Sensorineural hearing loss, or SNHL, occurs when sound-sensing hair cells in the cochlea, the auditory nerve, or the brain’s central auditory pathways are damaged. Unlike conductive hearing loss, which can result from earwax, infection, or problems in the middle ear, SNHL generally affects the biological systems responsible for converting sound vibrations into electrical signals. Because these structures have limited capacity for repair, the condition can be permanent and may progress silently before patients recognize a change in hearing.

The study involved 201 adults with hypertension who were enrolled between October 2023 and May 2024. Each participant completed a structured interview and underwent an ear examination using otoscopy, followed by pure-tone audiometry in a soundproof room. Pure-tone audiometry measures the softest sounds a person can hear at different frequencies and is widely used to identify the degree and pattern of hearing impairment. The researchers reported that 23.9% of the participants had SNHL.

The burden was not evenly distributed across the study population. Hearing loss was detected in 59.2% of participants older than 50, compared with 12.5% of younger adults. Women were also more frequently affected than men, with SNHL reported in 31.3% of female participants and 14.6% of male participants. The strongest pattern emerged when researchers examined how long participants had lived with hypertension: 64.2% of those with hypertension for at least five years had SNHL, compared with only 3.7% of those with a shorter history of the disease.

Most affected participants had hearing loss in both ears. Bilateral SNHL accounted for 81.3% of cases, suggesting that the condition may reflect systemic processes rather than an isolated problem involving one ear. The majority of cases were classified as mild, representing 70.8% of the hearing-loss group. Even mild impairment, however, can interfere with conversations, workplace communication, education, and social participation, particularly in noisy environments where speech sounds are difficult to separate from background noise.

The biological connection between high blood pressure and hearing damage is plausible. Hypertension can alter the small blood vessels that supply the cochlea, an organ that depends on a delicate and continuous blood flow. Increased blood viscosity and vascular narrowing may reduce cochlear perfusion, while prolonged ischemia can trigger oxidative stress and damage sensory hair cells. Hypertension may also impair the body’s ability to repair or maintain the tiny blood vessels of the inner ear. Because cochlear tissues have high metabolic demands, even subtle changes in circulation could potentially affect auditory function over time.

Smoking emerged as the strongest modifiable factor associated with SNHL in the analysis, with an odds ratio of 21.2. In epidemiological research, an odds ratio of this size indicates a particularly strong association, although it does not by itself prove that smoking caused the hearing loss. Tobacco smoke can damage blood-vessel function, promote inflammation, increase oxidative stress, and reduce oxygen delivery to vulnerable tissues. These mechanisms could intensify the effects of hypertension on the cochlea and auditory nerve. Alcohol use, female sex, older age, prolonged hypertension, and the use of diuretic medications, including furosemide and hydrochlorothiazide, were also associated with hearing loss.

The findings point to a potential role for hearing screening in long-term hypertension management. Patients with years of elevated blood pressure may benefit from hearing assessment even when they report only subtle symptoms, especially if they smoke, use alcohol heavily, or take medications that may affect auditory function. Clinicians should not discontinue medically necessary drugs without professional guidance, but long-term users of potentially ototoxic medicines may require careful medication review and monitoring. Early identification could allow patients to receive counseling, communication support, hearing devices, or referral to an ear, nose, and throat specialist before the impairment becomes more disruptive.

The researchers caution that their results should be interpreted carefully. The investigation was conducted at a single tertiary hospital and used a cross-sectional design, meaning participants were assessed at one point in time. This approach can reveal patterns and associations but cannot establish whether hypertension directly caused hearing loss, whether hearing impairment preceded hypertension, or whether other factors contributed to both conditions. The study population may also differ from patients in rural communities or other regions of Tanzania. Larger community-based surveys and prospective studies that follow patients over time will be needed to determine how hearing changes develop and whether improved blood-pressure control can reduce the risk.

Despite these limitations, the study highlights an important shift in how hypertension may be understood. The disease is commonly monitored through its effects on the heart, brain, kidneys, and blood vessels, while the inner ear receives far less attention. The Tanzanian findings suggest that hearing should be considered part of comprehensive chronic-disease care. Protecting hearing may require the same broad strategy used to protect other organs: early diagnosis, sustained blood-pressure control, smoking cessation, moderation of alcohol use, and careful prescribing. For millions of people living with hypertension, preserving the ability to hear may be another reason to treat the condition early and consistently.
