CLEVELAND, Ohio—Women living with type 2 diabetes may experience a heavier burden of menopause symptoms, according to a new observational study of nearly 300 Korean women between the ages of 40 and 64. The research, published online in Menopause, found that women classified in the diabetic group reported more menopause-related symptoms and greater symptom severity than women without diabetes. The differences were most pronounced—and statistically significant—among women who had reached postmenopause.
Menopause is not a single event but a biological transition shaped by changing ovarian hormone production. As estrogen levels fluctuate and eventually decline, women may experience hot flashes, night sweats, sleep disruption, mood changes, fatigue, cognitive complaints, joint discomfort, and genitourinary symptoms. The intensity and combination of these symptoms vary widely. The new findings suggest that glucose metabolism may be one factor influencing why some women experience a substantially more difficult transition than others.
The investigators assessed symptoms using the Midlife Women’s Symptom Index, a structured tool designed to measure both the presence and severity of symptoms associated with the menopausal transition. Participants were evaluated across different menopause stages, allowing the researchers to compare women in perimenopause, menopause, and postmenopause. The study also examined whether participants had type 2 diabetes or prediabetes, grouping these women together for analysis against women without diabetes-related glucose abnormalities.
Across every menopause stage, the diabetic group reported a higher prevalence of symptoms. Symptom severity was also consistently greater among women with diabetes or prediabetes. However, the statistical differences reached significance only in the postmenopause group. This means the observed pattern was unlikely to be explained by random variation in that stage, although the study does not establish that diabetes directly causes menopause symptoms or that diabetes treatment will eliminate them.
The relationship between diabetes and menopause is biologically plausible. During midlife, declining estrogen levels can contribute to changes in body-fat distribution, insulin sensitivity, and cardiometabolic risk. Fat may increasingly accumulate around the abdomen, where adipose tissue is metabolically active and can promote inflammation and insulin resistance. At the same time, chronic high blood glucose can affect blood vessels, nerves, immune signaling, and energy metabolism—systems that may also influence thermoregulation, sleep, sexual health, and mood.
Hot flashes and night sweats, known collectively as vasomotor symptoms, are controlled by neural circuits involved in body-temperature regulation. Estrogen withdrawal can narrow the thermoneutral zone, the range of internal temperatures that the brain tolerates without triggering sweating or shivering. Diabetes-related vascular dysfunction, inflammation, and altered autonomic nervous-system activity could potentially make these responses more noticeable or disruptive. These mechanisms remain hypotheses, however, and the current study was not designed to prove how diabetes might intensify individual symptoms.
The findings are particularly relevant as type 2 diabetes becomes more common with age. Global diabetes prevalence was estimated at approximately 10 percent in 2024, and midlife women face increasing risk as hormonal changes, weight gain, reduced muscle mass, and abdominal fat accumulation converge. Previous research has identified a two-way relationship between diabetes and menopause: early menopause and the postmenopausal years may be associated with greater diabetes risk, while metabolic disease may worsen the experience of menopause-related symptoms.
Earlier studies have often focused on menopause timing, age at menopause, or the stage of the transition rather than measuring the overall symptom burden. By directly comparing symptom number and severity in women with and without diabetes-related glucose abnormalities, the new research adds another dimension to the evidence. Still, its observational design imposes important limits. The researchers observed associations at a particular point in time, so they could not determine whether diabetes preceded worsening symptoms, whether symptoms contributed indirectly to metabolic deterioration, or whether both were influenced by factors such as obesity, medication use, sleep quality, depression, socioeconomic conditions, or access to healthcare.
The authors say the results support integrating menopause assessment into routine diabetes care for midlife women. Clinicians may need to ask specifically about vasomotor symptoms, sleep, mood, sexual and urinary health, and changes in daily functioning rather than assuming these complaints are an unavoidable part of aging. At the same time, the menopause transition can provide an opportunity to screen for blood pressure abnormalities, lipid disorders, elevated blood glucose, and other cardiovascular risk factors. Dr. Stephanie Faubion, medical director of The Menopause Society, noted that women with diabetes may carry a greater symptom burden and that frequent vasomotor symptoms are associated with a less favorable cardiometabolic profile.
Longitudinal studies following women before, during, and after menopause will be needed to clarify the direction of the relationship and determine whether improved glucose control changes symptom trajectories. Future research may also distinguish type 2 diabetes from prediabetes, examine specific symptoms rather than overall scores, and assess the influence of diabetes duration, medications, body composition, and glycemic variability. For now, the study’s central message is practical: menopause symptoms in women with diabetes should be recognized as clinically meaningful concerns, not dismissed as unrelated or inevitable, because addressing them may improve quality of life while revealing broader risks to women’s long-term health.
