Study Examines Link Between Neonatal Male Circumcision and Autism Diagnoses

A large US cohort study has found no evidence that neonatal medical male circumcision is associated with autism spectrum disorder (ASD) or with autism-related traits and behaviors. The analysis, conducted across 14 research sites in the Environmental Influences on Child Health Outcomes (ECHO) Cohort, examined 2,771 male children and addresses a question that has attracted public concern despite limited scientific evidence linking the procedure to later neurodevelopmental outcomes.

The study focused on neonatal medical circumcision, a surgical procedure performed shortly after birth to remove the foreskin from the penis. Researchers compared children who underwent the procedure with those who did not and assessed whether circumcision status was associated with an eventual ASD diagnosis or with behavioral and developmental characteristics related to autism. Their central finding was null: children who underwent neonatal medical circumcision did not show a higher likelihood of ASD diagnosis or autism-related traits and behaviors.

ASD is a neurodevelopmental condition involving differences in social communication, interaction, behavior, and sensory processing. Its origins are complex and involve a combination of genetic, biological, and environmental factors. Because ASD develops through multiple pathways, researchers must distinguish between a statistical association and a causal relationship. In this study, the absence of an observed association means that circumcision was not linked to increased ASD risk within the examined cohort; it does not establish that every possible circumstance surrounding the procedure has been studied or that observational research can prove causality.

The investigators used data from the ECHO Cohort, a nationwide research program designed to examine how early-life exposures influence children’s health and development. Cohort studies follow groups of participants over time, allowing researchers to compare later outcomes according to exposures recorded earlier in life. This approach can provide valuable evidence about developmental patterns, although results may still be influenced by differences between families who choose circumcision and those who do not, as well as by incomplete information about medical, social, or environmental factors.

To test whether the result was consistent across different clinical and geographic circumstances, the researchers conducted additional analyses. The findings remained robust after stratification by geographic region, preterm birth, and admission to a neonatal intensive care unit. These factors are important because premature birth and serious neonatal illness can be associated with differences in early medical care and neurodevelopmental outcomes. Examining them separately helped assess whether the overall result was being driven by one particular region or by children with more complex neonatal histories.

The study also considered acetaminophen exposure during the circumcision procedure or during the neonatal period. Acetaminophen is commonly used to manage pain or discomfort, and its potential relationship to neurodevelopment has been the subject of continuing scientific debate. In this cohort, acetaminophen use in connection with the procedure or during the neonatal period was uncommon, occurring in fewer than 5% of cases. The low frequency of exposure means the study provides limited information about acetaminophen itself, but it reduces the likelihood that widespread use of the medication explains the study’s main findings.

The authors’ conclusion is therefore narrower than a claim that circumcision has no health effects at all. The analysis specifically examined whether neonatal medical circumcision was associated with ASD diagnosis or autism-related traits and behaviors, and it found no evidence of such an association. Like all observational studies, the research cannot eliminate every possible source of confounding, including differences in family characteristics, healthcare practices, or reasons parents selected or declined the procedure. Nonetheless, the consistency of the findings across several subgroups strengthens the study’s main conclusion.

The results may be particularly relevant to families making decisions about neonatal circumcision while navigating conflicting information online. Claims that routine newborn procedures cause autism can spread rapidly when complex medical questions are reduced to simple cause-and-effect narratives. The new analysis does not resolve every debate surrounding circumcision, including questions about pain management, cultural practice, ethics, or other medical outcomes. It does, however, provide cohort-based evidence against an increased risk of ASD associated with neonatal medical circumcision.
