Third Course of Stereotactic Radiotherapy Shown Safe in Spinal Osteosarcoma Case

Doctors in Warsaw have reported what they describe as the first documented case of a second reirradiation with stereotactic body radiotherapy (SBRT) in a patient with metastatic osteosarcoma. The case report, accepted for publication in Frontiers in Oncology in the Radiation Oncology section, details how a third course of precisely targeted radiotherapy to the spine achieved a partial response without any acute adverse events, offering a rare data point in an area where evidence is extremely scarce. The patient, treated at the Maria Sklodowska-Curie National Research Institute of Oncology, had already undergone multiple surgical interventions in the cervical and thoracic spine and three lines of chemotherapy before showing local progression in the cervicothoracic region of the spine.

The multidisciplinary team then delivered a third SBRT course to the oligoprogressive spinal lesions. SBRT has emerged in recent years as a promising approach for vertebral metastases generally, achieving high local control rates with a favorable safety profile. Despite that growing trend, the authors note that evidence on spinal reirradiation with SBRT remains limited, and data are especially sparse for radioresistant histologies such as osteosarcoma, which is why this single case carries weight for the field.

The reported outcome was a partial response at the treated spinal sites, with no acute adverse events recorded. The authors conclude that even a third SBRT course can yield clinical benefit and be executed safely, provided it is delivered according to proper technical regimens and critical organ sparing strategies. They emphasize that this applies to a carefully selected group of patients, and that the finding comes from a single case rather than a controlled study.

The report also underscores the role of multidisciplinary decision-making in sarcoma care. For the field, the case adds to a small but growing literature on repeat stereotactic irradiation of the spine. Preliminary studies cited by the authors, together with this report, suggest the approach can be useful, effective and safe in selected patients, including those with histologies traditionally considered difficult to treat with radiotherapy.

The authors caution, however, that the evidence base is still preliminary. As a single-patient case report, the study's conclusions rest on one individual's outcome, and the authors themselves present it as a signal rather than a definitive result. Nevertheless, they present it as evidence that repeated SBRT to spinal metastases, even a third course, can be considered in carefully selected patients with radioresistant tumors, pending the larger datasets that will be needed to confirm who benefits most.

The work was carried out by a team from the Department of Soft Tissue/Bone Sarcoma and Melanoma, the Department of Pathology and the Department of Radiotherapy I at the Maria Sklodowska-Curie National Research Institute of Oncology in Warsaw, and the authors state that all claims expressed in the article are solely their own and do not necessarily represent those of their affiliated organizations or the publisher.
