School Vaccination Requirements Serve as Preventive Care Infrastructure

School-entry vaccine requirements may offer one of the most strategically important connections between public education and public health, according to a Viewpoint in JAMA Health Forum. The article examines how the administrative process used to determine whether children can attend school may also create a valuable opportunity to identify missed vaccinations, connect families with health services, and strengthen community protection against infectious disease.

The Viewpoint is authored by Y. Tony Yang, ScD, LLM, MPH, of the Center for Health Policy and Media Engagement at George Washington University. Its central premise is that schools are not merely educational institutions when it comes to vaccination. They are also places where nearly all children and families interact with a public system, creating a practical channel for health agencies and clinicians to reach populations that may otherwise remain outside routine medical care.

School-entry requirements generally involve documentation showing that a child has received vaccines recommended or mandated under state or local law. These rules are designed to reduce the likelihood that vaccine-preventable diseases will spread in classrooms, where students spend extended periods in close contact. From an epidemiological perspective, high vaccination coverage lowers the number of susceptible individuals in a population, reducing the effective reproduction of an infection and helping protect people who cannot be fully vaccinated for medical reasons.

The administrative infrastructure behind these requirements can also function as an early-warning and intervention system. When vaccination records are incomplete, outdated, or unavailable, schools may be able to notify families before an outbreak occurs. That notification can prompt a visit to a pediatrician, community health center, pharmacy, or local health department. In this way, a policy intended primarily to protect the school environment may also help close gaps in preventive care.

This connection is especially important because children do not all have the same access to regular health services. Some families face financial barriers, transportation problems, limited clinic availability, language obstacles, or difficulties navigating insurance and appointment systems. Others may move frequently, change health care providers, or lack complete medical records. A school-based verification process can reveal these gaps, but its public health value depends on what happens afterward.

The Viewpoint highlights the need for coordination rather than treating schools and health care organizations as separate systems. Secure data-sharing arrangements, standardized immunization records, clear referral pathways, and adequate staffing could allow schools to identify students who need follow-up without turning educators into health care providers. Health departments and clinical organizations could use these signals to offer accessible vaccination services, including outreach through school-linked clinics, mobile programs, pharmacies, or community events.

Such coordination must address technical and ethical challenges. Immunization records contain protected health information, and any exchange of data must comply with applicable privacy rules and institutional policies. Systems must also be interoperable: an electronic record created by one clinic should be readable and accurately matched to a child’s record in another system. Poorly designed databases can produce duplicate entries, missing doses, or incorrect flags, potentially leading to unnecessary revaccination, delayed enrollment, or avoidable conflict with families.

The policy environment is also complex. School-entry mandates differ across jurisdictions in the vaccines covered, the timing of required doses, medical exemptions, and procedures for temporary exclusion. Enforcement can increase vaccination rates, but punitive approaches may disproportionately affect families with limited resources. A technically effective strategy therefore requires more than a legal mandate. It must combine clear communication, convenient access to vaccination, culturally responsive outreach, and assistance for parents who encounter administrative or medical barriers.

The Viewpoint’s broader message is that school-entry vaccination policies should be understood as a public health platform as well as a compliance mechanism. By linking education records, health departments, clinicians, and families in a carefully governed system, communities may be able to improve routine immunization, detect care gaps earlier, and reduce the risk of outbreaks. The approach does not eliminate the need for trusted medical advice or robust health care access, but it could turn an existing administrative interaction into a more effective bridge between two systems that serve nearly every child.
