A seven-week program delivered through ordinary physical education classes in Spanish secondary schools has shown that even brief, structured sessions can measurably ease the psychological pressures that social media and cultural ideals place on adolescent bodies. The intervention, called RealBody+, was developed and tested by researchers at the University of Granada and the International University of La Rioja, and is described in a new study published in the journal School Mental Health. Its central premise is deceptively simple: if teenagers can be taught to recognize the gendered body stereotypes that circulate online, and given space to explore how their bodies move and express themselves, the internal grip of those ideals may loosen.
The research team enrolled 250 students, 113 boys and 137 girls aged 11 to 17, drawn from secondary education and the first year of Baccalaureate. The study used a quasi-experimental design with non-equivalent groups. One hundred twenty-one students were assigned to the experimental group, which completed seven one-hour sessions embedded within Physical Education classes. These sessions combined body expression activities, essentially structured movement and improvisation exercises designed to build comfort and familiarity with one’s own body, with targeted exercises aimed at raising awareness of online gendered body stereotypes. The remaining 129 students formed an active control group that attended informational lectures on related topics instead.
The technical logic of the design matters. Because both groups received some form of attention and content, the comparison tested not simply whether any intervention helps, but whether the specific blend of embodied movement and digital-stereotype awareness outperforms a more conventional didactic approach. The researchers measured three psychological variables before and after the program: internalization of gender-specific body ideals, meaning the degree to which students personally endorsed thinness ideals for girls and muscularity ideals for boys; social appearance anxiety, the fear of being negatively judged on one’s looks; and symptoms of disordered eating. Standardized psychometric instruments were administered at both time points, allowing the team to compute changes within each group and, critically, group-by-time interaction effects.
The baseline data painted a familiar but sobering picture of adolescence in the digital era. Girls reported significantly higher media pressure than boys, consistent with a large body of research showing that girls absorb more appearance-related messaging and face more intense scrutiny of their bodies online. Perhaps more striking, boys were far from immune: students of both genders reported that their primary motivation for exercise was to modify their appearance rather than to build fitness, health, or enjoyment. This finding underscores how deeply appearance-driven reasoning penetrates adolescent physical activity, shaping behavior in ways that can undercut the very health benefits exercise is supposed to provide.
After the intervention, the results were encouraging, though with an important twist. Both the experimental group and the active control group showed statistically significant improvements across the measured variables. Students in both conditions reported reduced internalization of gendered body ideals, lower social appearance anxiety, and fewer eating disorder symptoms. However, the analysis found no significant group-by-time interaction effects. In plain terms, the carefully constructed combination of body expression and digital literacy work did not outperform the simpler lecture-based format in this sample. Both pathways appeared to move the needle by roughly similar amounts.
The researchers are careful in their interpretation, and the honesty of that interpretation is itself notable. The RealBody+ program was associated with improvements in psychological aspects of body perception, including reduced internalization of gendered body ideals, social appearance anxiety, and eating disorder symptoms. But because the changes were comparable across both groups, the study cannot claim that the specific active ingredients of the experimental format, the movement work or the stereotype-awareness exercises, were uniquely responsible. It is possible that simply dedicating structured classroom time to talking about bodies, media, and health produces most of the benefit, and that the precise format matters less than educators have assumed.
That conclusion carries real practical weight. School-based body image interventions have historically struggled with scalability. Programs requiring trained specialists, extended curricula, or costly materials face steep barriers to adoption, particularly in under-resourced schools. If a lecture-based format delivered within existing Physical Education time produces comparable gains to a more elaborate program, schools have a cheaper, more flexible menu of options. Conversely, if future research shows that the experimental format shines under different conditions, for example with longer follow-up, different age bands, or students with higher baseline risk, the movement-based approach may still earn its place. The current study, by including an active rather than passive control group, set a demanding bar for itself.
The broader scientific context reinforces why this line of work matters. Decades of research have linked internalization of thin and mesomorphic ideals to disordered eating, compulsive exercise, depression, and social withdrawal in adolescents. The rise of image-centric social media has intensified these pressures: content analysis studies of fitspiration imagery, selfie-editing behavior, and appearance-focused platforms document a steady stream of gendered body messaging that adolescents consume daily. Objectification theory provides the dominant explanatory framework, holding that internalizing an observer’s perspective of one’s own body fosters habitual body monitoring, appearance anxiety, and, downstream, risky eating and exercise behaviors. Girls appear to absorb more of this pressure on average, but the Granada baseline data on appearance-driven exercise motivation among boys confirms that muscularity ideals now exert a parallel toll.
What distinguishes RealBody+ from many predecessors is its dual-channel design. Rather than treating body image as purely a matter of cognition, something to be corrected with facts and counter-arguments, the program also engages the body directly through expression activities. The rationale draws on pedagogical traditions in physical education that view embodiment, comfort with movement, and non-verbal self-expression as protective resources. At the same time, the digital literacy component acknowledges that the primary delivery system for harmful body ideals today is the smartphone screen, not the magazine rack. Teaching students to name and critique online gendered body stereotypes is, in effect, an inoculation strategy against the media environment they inhabit.
For now, the headline finding is both modest and useful: two different school-based approaches, one embodied and digitally critical, one informational, both produced significant short-term reductions in the psychological risk factors that precede eating disorders and body dissatisfaction. The study was supported by the University of Granada, the Spanish Ministry of Universities, and Spanish research excellence programs, and was approved by a local ethics committee in accordance with the Declaration of Helsinki. The authors note no conflicts of interest. Whether the gains persist, and whether combining the two formats yields additive benefits, remain open questions for future trials. But for schools searching for feasible ways to defend their students’ mental health against the pressures of the feed, the message from Granada is that structured classroom time spent on bodies, media, and health, in almost any thoughtful form, is time well spent.
The measurement toolkit used in the study reflects the standard instruments of the field. Eating disorder symptoms were assessed with the Eating Disorder Examination Questionnaire, a widely validated self-report measure originally developed by Fairburn and Beglin, while self-esteem was captured with the Spanish adaptation of the Rosenberg scale. Prior Spanish validation work on body esteem and appearance anxiety measures in adolescent samples has made such cross-sectional assessment feasible in school settings, which is one reason Spain has become an active site for preventive body image research.
The study also sits within a growing international evidence base. Comparable classroom programs, such as the five-session teacher-delivered body image curriculum evaluated in a cluster randomized trial and brief digital-literacy interventions tested with adolescents in the United Kingdom, have similarly reported reductions in body dissatisfaction when embedded in normal school timetables. The Spanish team’s choice of an active control group mirrors the methodological caution seen in these trials, where attention effects and simple psychoeducation often account for more benefit than researchers initially expect.
Several limitations temper the findings. The quasi-experimental design with non-equivalent groups means students were not randomly assigned, leaving open the possibility of baseline differences between conditions that influenced the pattern of change. The absence of a long-term follow-up assessment also means the durability of the improvements, whether students maintained lower appearance anxiety and fewer disordered eating symptoms months later, cannot yet be judged. Sample composition, drawn from Spanish secondary schools and spanning a wide age range from 11 to 17, may mask developmental differences in how younger and older adolescents respond to stereotype-awareness content.
Even so, the consistency of improvement across both conditions strengthens the case that schools can serve as effective delivery platforms for preventive mental health work, particularly when sessions fit within existing curricular structures rather than competing with them.
