Remdesivir improves outcomes in adult kidney transplant recipients with COVID-19

Remdesivir may offer benefits that extend well beyond the initial days of COVID-19 in adults living with kidney transplants, according to a target trial emulation analysis published in JAMA Network Open. The study evaluated whether timely antiviral therapy could influence longer-term outcomes in a population often excluded from randomized trials.

Researchers focused on adults treated at five Johns Hopkins Medicine hospitals in Maryland. Participants were diagnosed with COVID-19 between March 2020 and January 2024 and either received remdesivir within one week of diagnosis or did not receive the antiviral. To mirror treatment duration, only those who completed at least three consecutive days of remdesivir were included in the treated group.

The investigators intentionally excluded patients who received other COVID-19 therapies such as monoclonal antibodies or additional antivirals, aiming to isolate remdesivir’s association with later health events. The cohort comprised 432 adult kidney transplant recipients, with 177 (41%) receiving the remdesivir regimen and 255 (59%) receiving no remdesivir.

Clinical follow-up extended for up to one year. The primary endpoint combined graft failure and death from any cause, creating an “all-cause graft loss” outcome. Secondary outcomes included cardiovascular events, all-cause mortality, and the development of long COVID, defined using documented clinical reports.

Across the entire cohort, 11% experienced all-cause graft loss, while 10% developed cardiovascular complications such as myocardial infarction, stroke, or cardiac-related death. Death from any cause occurred in 8%, and long COVID was documented in 4% of participants, limiting statistical certainty for that endpoint.

After accounting for differences in age, comorbidities, medication use, transplant timing, COVID-19 severity proxies, vaccination status, and diagnostic period, early remdesivir treatment correlated with markedly lower risks. The treated group showed a 47% reduced risk of all-cause graft loss and a 42% reduced risk of cardiovascular events compared with those who did not receive remdesivir.

The study population carried substantial baseline risk. Many participants required supplemental oxygen at diagnosis, and large proportions had conditions such as hypertension, diabetes, and coronary artery disease. More than half also stopped immunosuppressive therapy at the time of COVID-19, reflecting a real-world management challenge during infection.

Biologically plausible mechanisms may connect early viral suppression to downstream effects on systemic inflammation and vascular injury. By potentially reducing endothelial damage and thrombotic pathways, remdesivir could help prevent events that contribute to both graft deterioration and cardiovascular complications.

This observational design cannot prove causality, but the findings support the hypothesis that antiviral timing matters in immunologically complex transplant patients, where both viral replication and host immune dysregulation can reshape risk trajectories.
