New Framework Links Human-Centered Design to Digital Health Implementation

Digital health tools promise better access, quality, and efficiency in care, yet many interventions stall in pilot phases or fail to achieve sustainable implementation. A new study argues that the problem often lies in how these tools are built and introduced: human-centered design has improved the usability and acceptability of digital health technologies, but its application frequently remains confined to early design activities and is rarely integrated with implementation science across the full implementation lifecycle.

To address this gap, researchers developed a multi-level, human-centered design framework informed by a cross-project methodological synthesis that integrates participatory design and implementation science. The work, published in Frontiers in Digital Health, aimed to support the design, implementation, evaluation, and scale-up of digital health interventions in a single, structured approach.

The synthesis drew on complementary digital health implementation studies undertaken in Kenya, spanning five distinct domains: adolescent mental health, neonatal clinical decision support, digital health infrastructure, generative artificial intelligence, and frugal digital health innovation. Framework development followed an iterative analytical process combining thematic synthesis, constant comparative analysis, and abductive reasoning, organizing recurring methodological themes into an integrated framework spanning user, workflow, and system perspectives across the implementation lifecycle.

From this analysis, the researchers identified recurring methodological processes that consistently contributed to successful digital health implementation. These included contextual understanding, participatory stakeholder engagement, iterative refinement, workflow integration, organizational readiness, governance, and sustainability. Rather than treating these as isolated steps, the framework positions them as processes that must operate together as interventions move from concept to routine use.

The resulting framework is structured around six iterative implementation phases—Understand, Co-design, Prototype, Evaluate, Implement, and Scale—and three interconnected analytical levels: the user level (micro), the workflow level (meso), and the system level (macro). The framework conceptualizes digital health implementation as a continuous sociotechnical process in which sustained adoption and scale-up depend upon coordinated alignment among these three levels throughout the implementation lifecycle.

This multi-level structure is central to the study’s argument. Interventions that are designed around user needs but ignore organizational workflows, or that fit local workflows but lack governance and system-level support, are unlikely to achieve durable adoption. By requiring alignment across micro, meso, and macro levels at every phase, the framework treats implementation not as a handoff from designers to implementers but as an ongoing process of sociotechnical adjustment.

The authors position the work as an advance for human-centered design, achieved by integrating it with implementation science through an empirically derived, lifecycle-oriented methodological framework. Because the framework was synthesized from studies conducted in Kenya, including low-resource settings, it emphasizes context-responsive implementation rather than one-size-fits-all deployment.

The proposed framework may provide researchers, developers, practitioners, and policymakers with a structured approach for designing, implementing, evaluating, and scaling digital health interventions while supporting sustainable, context-responsive, and equitable digital health transformation. As a methodological synthesis, the study presents a framework derived from recurring processes across the included projects; its value for future implementations will depend on how it performs when applied in practice.
