A new randomized trial published in The Annals of Family Medicine tests a simple but powerful idea: when patients struggle to navigate the social and health services system, structured help may work better than pointing them to a website or hotline. The study evaluates how “social prescribing” support is delivered—through either an intensive, in-person navigation model or traditional signposting.

In the experiment, 326 patients in Ontario, Canada were randomly assigned to one of two approaches. The intervention, known as the Access to Resources in the Community (ARC) model, paired patients with a navigator who met them in person, mapped their needs to service options, and co-created an engagement plan aligned with patient priorities.

The control condition used signposting. Participants were directed to 211 Ontario, a free, online and multilingual phone service. Importantly, this resource primarily provides information during the initial call, with no planned follow-up to ensure that patients act on recommendations or that services match their evolving needs.

Three months after enrollment, the results favored ARC. Patients in the navigation arm reported significantly greater access to needed resources: 50.3% versus 35.8% in the signposting group. Beyond the access metric, the study also found that ARC participants reported a better overall experience and stronger capacity to engage with their care over time.

A key equity dimension emerged for Francophone minority patients. Because the system can be language-dependent, the ARC model improved access specifically to French-language services. This finding suggests that navigation intensity may help reduce practical barriers that disproportionately affect minority language communities.

The trial also speaks to “intervention intensity.” By comparing a longitudinal, relationship-based model with a single-pass information referral, the study provides evidence that how support is delivered can matter as much as the support itself.

For primary care systems under pressure, the findings point toward a scalable target: comprehensive navigation that actively translates patient needs into real-world resource connection. While the study does not imply every context needs the same level of staffing, it underscores the potential returns of moving from information delivery to structured support.

Overall, the trial offers viral-science style momentum for health innovation: when health care treats navigation as clinical infrastructure—rather than an optional add-on—patients may actually reach the resources they need.
