A diet rich in antioxidants was associated with a substantially lower prevalence of cervical cancer in a population-based study of women in the United States, according to research published in the International Journal of Gynecology & Obstetrics. The investigators reported that women with the highest scores on a measure of dietary antioxidant intake had a 55.8% lower prevalence of cervical cancer than those with the lowest scores. Although the findings do not establish that antioxidants prevent cervical cancer, they add to growing evidence that overall dietary patterns may influence the biological environment in which cancer develops.
The study analyzed health and nutrition data collected between 2003 and 2018 through the National Health and Nutrition Examination Survey, or NHANES. This ongoing US survey combines interviews, dietary assessments, physical examinations and laboratory testing to produce a detailed picture of population health. For the analysis, researchers examined information from 1,044 women, including 174 participants with cervical cancer and 870 without the disease. The team then investigated whether cancer prevalence was related to participants’ scores on the Composite Dietary Antioxidant Index, commonly known as the CDAI.
The CDAI is a composite measure designed to estimate the antioxidant potential of a person’s diet. Rather than focusing on one nutrient or one food, the index incorporates intake levels of several dietary components with antioxidant properties, based on information gathered during dietary-recall interviews. Antioxidants include substances such as vitamins and minerals, as well as other plant-derived compounds, that can neutralize or modulate reactive molecules generated during normal metabolism and inflammation. Because oxidative stress can damage DNA, proteins and cell membranes, researchers have proposed that antioxidant-rich diets might influence processes involved in cancer initiation and progression.
After adjusting the analysis for other factors that could affect cervical cancer prevalence, the investigators found that each one-unit increase in the CDAI was associated with an 11.4% lower prevalence of cervical cancer. When participants were divided into three groups according to their CDAI scores, the women in the highest tertile had a 55.8% lower prevalence than women in the lowest tertile. The analysis also indicated a linear inverse relationship, meaning that cervical cancer prevalence tended to decrease as the antioxidant index increased rather than showing a clear threshold at which the association suddenly appeared.
Cervical cancer most often begins with persistent infection by high-risk types of human papillomavirus, or HPV. In many people, the immune system clears the infection, but persistent HPV can cause cellular changes in the cervix that may progress from precancerous lesions to invasive cancer. Oxidative stress and chronic inflammation have been investigated as possible contributors to this progression. Reactive oxygen species can create chemical changes in DNA and may affect signaling pathways that regulate cell division, immune responses and programmed cell death. A diet containing a broad range of antioxidant-rich foods could theoretically influence these pathways, although the precise mechanisms remain uncertain.
The researchers emphasized that the study shows an association rather than a cause-and-effect relationship. Its cross-sectional design means that dietary intake and cancer status were assessed within the same general period, making it impossible to determine whether a lower antioxidant intake preceded the development of cervical cancer. Participants may also have changed their diets after receiving a diagnosis, and dietary-recall interviews can be affected by imperfect memory or differences in portion estimates. In addition, a composite dietary score may reflect more than antioxidant exposure alone: people with higher CDAI values may also consume more fruits, vegetables and whole grains, have different levels of physical activity, or experience other social and health-related advantages.
The results therefore should not be interpreted as evidence that antioxidant supplements can prevent cervical cancer. Nutrients consumed in food are part of complex dietary and metabolic systems, and isolated supplements may not reproduce the effects of whole foods. Some high-dose supplements have produced unexpected or harmful outcomes in other disease settings, particularly when taken without a demonstrated deficiency. The study also did not show that a specific fruit, vegetable, vitamin or supplement was responsible for the observed association. Instead, the findings point to the potential importance of overall dietary antioxidant exposure as one component of cervical cancer risk research.
The most effective established strategies for reducing cervical cancer risk remain HPV vaccination, regular screening and timely treatment of precancerous abnormalities. HPV vaccination can prevent infection with virus types responsible for most cervical cancers, while screening methods such as HPV testing and cytology can identify cellular changes before they become invasive disease. These interventions are especially important because cervical cancer is often preventable when prevention, screening and follow-up care are accessible. Dietary quality may eventually be shown to complement these measures, but it cannot replace vaccination, screening or medical treatment.
The study’s authors called for prospective investigations that follow women over time and record dietary intake before cancer develops. Such studies could clarify whether higher antioxidant exposure is linked to a lower risk of persistent HPV infection, progression of cervical precancer or invasive cancer itself. Researchers will also need to determine which foods and antioxidant compounds are most relevant, how dietary patterns interact with immune function, and whether the relationship differs according to age, HPV status, smoking history or access to screening. For now, the findings provide a provocative population-level signal: women reporting diets with greater overall antioxidant content had lower cervical cancer prevalence, but confirming whether nutrition can alter the course of cervical disease will require carefully designed longitudinal and mechanistic research.
