A theoretical review of adolescent mental health proposes that mental pain is a multidimensional process involving emotional distress, social disconnection, cognitive overload and difficulties in self-regulation. The framework is designed to explain why adolescents exposed to similar stressors can experience markedly different psychological outcomes, including depression, suicidal behavior and non-suicidal self-injury. Rather than treating distress as a single symptom, the model frames it as an imbalance between emotional demands and regulatory capacity within a social and relational context.

The review, published in Annals of General Psychiatry, integrates empirical and theoretical literature on adolescent development, emotion regulation, developmental neuroscience, suicidality, social context and digital environments. The authors give priority to recent longitudinal and clinical studies, while also drawing on foundational theoretical models. This is an overview and proposed framework rather than a new experiment or clinical trial, so its contribution is conceptual: it connects developmental vulnerability, emotion regulation, affective control and mental pain tolerance in an attempt to account for variation in adolescent outcomes.

At the center of the proposed model is emotion regulation, which the review describes as a transdiagnostic mechanism operating across forms of psychopathology. Affective control is presented as the cognitive foundation that supports emotion-regulation processes. The framework also distinguishes mental pain intensity from mental pain tolerance and identifies tolerance as a critical moderator of developmental and clinical outcomes. This distinction means that the overall experience of suffering and the capacity to withstand it are treated as separate features that may influence how distress develops. The review suggests that differences in regulatory capacity, distress tolerance and social and relational determinants may help explain why comparable stressors do not translate into identical experiences of distress or the same behavioral outcomes.

Environmental conditions are placed within this developmental and ecological framework rather than treated as isolated causes with automatic effects. Pandemic-related disruptions, rapidly changing sociocultural conditions and digitally mediated experiences may amplify or attenuate psychological distress, depending on their interaction with developmental vulnerability. The framework also considers relational connectedness, supportive social environments and existential concerns as factors that can shape the experience of unbearable psychological pain. These elements are presented as parts of a dynamic system in which internal capacities and external conditions interact. The review concludes that attention to adolescent mental pain should include both the causes of intolerable psychological suffering and the capacities for emotion regulation, distress tolerance and social connection. It does not report that a particular intervention prevents self-harm or suicidal behavior, nor does the supplied evidence establish the model as a clinically validated tool.

Subject of Research: Developmental-ecological framework for adolescent mental pain and variable psychological outcomes.
