Changes in activities of daily living (ADL) were positively associated between spouses in a longitudinal cohort of 3,425 married couples in Shanghai, according to the study’s published abstract. The analysis also found that the severity of one partner’s ADL disability at baseline was associated with a lower ADL change score for the other partner, while the study found no statistically significant overall difference in mean spousal ADL change among four caregiving configurations.

The study followed 6,850 older adults from 2018 to 2021. ADL change for each partner was calculated as the final available ADL score minus the baseline score. Researchers used reciprocal multivariable linear regression models to examine associations between partners’ ADL changes and selected demographic, socioeconomic, caregiving and residential-environment characteristics. Mean spousal ADL change across caregiving subgroups was compared using Welch’s one-way analysis of variance. The available evidence is an abstract, so it does not provide the full survey measures, detailed covariate definitions or complete participant-flow information.

In the reciprocal models, partners’ ADL score changes were positively associated (β = 0.164, p < 0.001). Compared with participants without baseline ADL disability, mild disability was associated with a 1.789-point lower spousal ADL change score (95% CI −1.998 to −1.580, p < 0.001). Moderate or severe disability was associated with a 4.044-point lower score (95% CI −4.483 to −3.605, p < 0.001). The summary does not establish how the underlying ADL instrument was scored, so the positive coefficient should not be translated into a specific direction of functional improvement or decline.

Formal care hours were positively associated with spousal ADL change (β = 0.004, 95% CI 0.003 to 0.005, p < 0.001), whereas the association with informal care hours was not statistically significant (β = −0.003, 95% CI −0.005 to 0.000, p = 0.074). Although mean spousal ADL change differed descriptively across the care groups, the overall between-group difference was not statistically significant (Welch F(4, 51.34) = 1.97, p = 0.114). These observational results identify associations rather than causal effects and do not demonstrate that either type of care produced a particular functional trajectory or that one caregiving configuration was superior. The study’s scope was also limited to older married couples followed in one Chinese city, with the available abstract describing 3,425 couples rather than the broader older-adult population. The authors concluded that functional changes were correlated within older couples and supported considering both partners when assessing functional health, without establishing causality or identifying a preferred care arrangement.
