A mandatory medical student research program produced at least one research output for 487 of 2,451 students, and a 14-week protected-block model was associated with higher odds of output than a longitudinal model with unprotected research time. The retrospective longitudinal cohort study covered nine student cohorts from 2014 to 2024 and searched publication records through January 2026. Participants generated 597 unique outputs: 536 journal articles, 53 conference presentations and eight other research products.

Researchers used student–supervisor surname combinations in Google Scholar to identify outputs related to students’ projects, including peer-reviewed journal articles, conference presentations and other formats. Among the journal articles for which journal-level data were available, 93% appeared in journals classified as Q1 or Q2. Their median Journal Impact Factor was 3.6, with an interquartile range of 2.7 to 4.9.

The median publication lag was 1.0 year, with an interquartile range of 1.0 to 2.0 years. Publication lag became shorter across successive cohorts, a difference the researchers assessed with a Kruskal–Wallis test that yielded H = 82.0 and p < .001. The projects also covered 13 Fields of Research divisions, indicating a broad disciplinary scope rather than concentration in one area.

Students occupied the first-author position in 48% of student–output pairs. The proportion of students with at least one output was 20%, which the study described as consistent with the 10% to 30% publication rates reported internationally for students in structured research programs. Binary logistic regression was used to examine factors independently associated with research output.

In the primary analysis, covering cohorts 1 through 8, the protected-block model was associated with an odds ratio of 1.82 and a 95% confidence interval of 1.44 to 2.30. No measured student demographic or background characteristic was independently associated with output. It also persisted when researchers restricted the outcome to peer-reviewed journal publication, for which the odds ratio was 1.64 and the 95% confidence interval was 1.29 to 2.09.

These findings do not establish that protected time alone caused the difference in output. Program structure was inseparable from cohort and calendar period, meaning changes in output cannot be cleanly attributed to the 14-week model rather than differences between cohorts or changes over time. The retrospective publication search also depended on the discoverability of outputs and the decisions used to match them to student projects.

Accordingly, the results support an association between protected research opportunities and greater scholarly output within this mandatory program, but they do not demonstrate that protected time independently raises publication rates across medical students generally.
