An operational evaluation of eight disease outbreaks in Mardan district, Khyber Pakhtunkhwa, Pakistan, found that only two outbreaks, or 25%, met all three targets of the 7-1-7 framework: detection within seven days, notification within one day and initiation of response within seven days. The study examined outbreaks recorded from 2022 through 2024 and assessed performance within the Early Warning System component of the Integrated Disease Surveillance and Response strategy. The study used an explanatory sequential mixed-methods design.

It first analyzed eight outbreaks retrospectively and then conducted 18 semi-structured, in-depth interviews to examine operational bottlenecks and enablers at the detection, notification and response stages. The outbreak sample comprised three dengue fever events, two measles events, two cholera or acute watery diarrhoea events and one leishmaniasis event. Detection was the first timing stage to fall short of the framework’s target.

The median detection interval was nine days, with an interquartile range of seven to 11 days. Notification took a median of two days, with an interquartile range of one to two days, compared with the one-day notification target. The median response interval was nine days, with an interquartile range of eight to 11 days, exceeding the target for initiating response within seven days.

Interviews attributed detection delays mainly to centralised laboratory confirmation. The findings indicate that reliance on laboratory results presented an operational constraint for identifying outbreaks within the seven-day target. The investigators did not present the interview evidence as proof that a single factor caused the measured delays, but the analysis identified centralised confirmation as the principal reported contributor to detection delays.

Reporting arrangements also created barriers at the notification and response stages. Participants described fragmented reporting and limited connectivity as constraints on communicating information and initiating action. Staff motivation and mobile-based reporting were identified as enablers in the operational system, although the study did not establish that either factor independently produced faster notification or response.

The findings describe the outbreaks and interviews included in this district-level evaluation, rather than the performance of Pakistan’s entire disease-surveillance system. The retrospective component covered a small number of outbreaks across four disease categories, and the mixed-methods design combined quantitative timing measures with qualitative accounts of operational processes. The reported results should therefore be understood as an assessment of the eight outbreaks studied and the 18 people interviewed, not as a national estimate of compliance with the 7-1-7 targets.

The 7-1-7 target framework was developed by Resolve to Save Lives and later adopted by the World Health Organization and the US Centers for Disease Control and Prevention. Its purpose is to provide a performance-based metric for epidemic preparedness, with measurable goals for detecting, notifying and responding to outbreaks. Mardan’s evaluation translated those goals into indicators that could be assessed against the timing of real outbreak events.

Based on the observed intervals and the operational barriers described by participants, the study concluded that Pakistan’s Early Warning System–Integrated Disease Surveillance and Response system showed readiness to operationalize the 7-1-7 framework but needed stronger laboratory capacity, integrated digital reporting and improved intersectoral coordination. The authors further concluded that embedding 7-1-7 indicators into national monitoring of the Integrated Disease Surveillance and Response strategy would support compliance with the International Health Regulations (2005). The evaluation itself did not test the effect of that proposed policy change.

For outbreak preparedness, the study’s clearest measured result is that the three timing targets were met together in only one quarter of the eight outbreaks examined. Detection and response intervals were both centered at nine days, while notification was closer to its target. The operational evaluation points to laboratory confirmation, reporting fragmentation and connectivity as areas requiring attention if the framework is to be used consistently as a performance benchmark.
