										<content:encoded><![CDATA[<p>When the COVID-19 pandemic closed schools and kept children apart, many researchers and parents assumed that head lice would suffer the same fate as influenza and other close-contact infections. Lockdowns, it seemed, offered a rare chance for the parasite to be squeezed out of communities entirely, as isolated households cleared infestations without fresh introductions from classmates. Early reports appeared to confirm this: clinics in several countries recorded fewer cases, sales of pediculicidal treatments fell, and some surveys suggested that infestation rates among schoolchildren had dropped below pre-pandemic levels. A new narrative review, however, argues that this tidy story does not survive a careful re-examination of the evidence.</p>
<p>The review, published in Acta Parasitologica by medical entomologist Ian F. Burgess of the Medical Entomology Centre at Insect Research &amp; Development Limited in Cambridge, United Kingdom, revisited and re-evaluated all of the literature linking COVID-19 with head lice. Its conclusions are deliberately contrarian. Despite the increased opportunity for households to eliminate infestations during periods of isolation, some families with no outside contacts reported persistent head louse infestations throughout lockdowns. More strikingly, the review suggests that most reported changes in head louse prevalence were relatively short lived, or in some cases entirely illusory, and that in low- and middle-income countries lockdowns produced little or no measurable difference in prevalence at all.</p>
<p>To understand why the pandemic&#8217;s effect on lice was so much weaker than expected, it helps to consider the biology and epidemiology of the parasite itself. The head louse, Pediculus humanus capitis, is an obligate ectoparasite that spends its entire life on the human scalp, cementing eggs to hair shafts and feeding several times a day. Transmission occurs almost exclusively through direct head-to-head contact, which is why children of primary school age, who play and sit closely together, bear the greatest burden of infestation worldwide. Unlike airborne pathogens, lice cannot spread through casual proximity in a classroom; they need sustained contact, typically minutes of touching heads. That makes them exquisitely sensitive to changes in social behaviour, but also means that the household, not the school, can serve as a reservoir that keeps infestations alive even when external contacts are severed.</p>
<p>This reservoir effect is central to Burgess&#8217;s revised interpretation. Previous work by the same author had already suggested that some families act as persistent reservoirs of head lice within communities, maintaining infestations across generations of the parasite and repeatedly reintroducing them to schools after treatment campaigns. During the pandemic, the paradoxical reports from households with no outside contacts that continued to experience infestations fit this pattern. If lice were already established within a household, lockdown provided no escape: family members continued to share beds, sofas and close contact, allowing transmission to continue unabated. In such households, the pandemic may even have prolonged infestations, because access to treatment products, professional services and school-based screening was disrupted.</p>
<p>The evidence for a pandemic-driven decline in lice came from several distinct types of data, each with its own weaknesses. Sales figures for pediculicidal treatments fell in several countries beyond the longer-term decline in product sales that had been observed for years before the pandemic, a trend driven largely by widespread insecticide resistance and the shift toward physical methods such as wet combing and silicone-based products. In France, a population-based study reported changes in the dynamics of head lice and scabies infestations during the pandemic, while researchers in Israel compared infestation rates before and during the epidemic and found reductions in some settings. In Buenos Aires, a survey of pediculosis during lockdown documented a decrease in infestation among children. Meanwhile, studies from Poland examined prevalence among children attending schools and kindergartens, and pharmacy sales data from Portugal and Poland were analysed for pandemic-era shifts in consumption of self-care products for pediculosis capitis.</p>
<p>Burgess&#8217;s re-evaluation questions how much of this apparent decline was real and how much was artefact. Some of the reported reductions were transient, rebounding as schools reopened and children resumed normal social lives. Others may have reflected changes in diagnosis and reporting rather than genuine changes in prevalence. During lockdowns, routine school screening programmes were suspended, parents had less access to professional diagnosis, and cases that would normally have been detected went unrecorded. Internet search data, which some researchers used as a proxy for infestation activity, capture public interest rather than true prevalence, and searches for head lice in the United Kingdom after the pandemic showed patterns that are difficult to interpret as straightforward measures of infestation. Overdiagnosis and mismanagement of head louse infestations were already well documented problems before the pandemic, with many nits, empty egg cases, mistaken for active infestations, so any decline in consultations could partly reflect reduced opportunities for misdiagnosis rather than fewer live lice.</p>
<p>The geographic pattern of the findings is particularly revealing. In low- and middle-income countries, where head louse prevalence among primary school children is often high and where lockdowns were shorter, less strictly enforced, or economically impossible to sustain, there was little or no difference in prevalence as a result of lockdowns. A systematic review and meta-analysis of prevalence and associated factors among primary school children in these countries documented persistently high infestation rates, and regional studies from places such as southern Iran found no dramatic pandemic-era interruption of pediculosis. In wealthier Western countries, by contrast, where prolonged school closures, furlough schemes and strict social distancing were more feasible, some communities did experience genuine, if temporary, reductions. The longest school closures of the pandemic, documented in global monitoring by UNESCO and Statista, lasted many months, and UNICEF estimated that at least one in seven children and young people lived under stay-at-home policies for most of a year, restrictions that would plausibly have curtailed head-to-head contact.</p>
<p>Yet even in Western settings, the recovery of louse prevalence has been slower than expected, and this is where the review offers its most intriguing conclusion. Some aspects of the social distancing imposed during the pandemic appear to have had lasting effects on children&#8217;s social interactions, particularly in Western countries, with a commensurate slower recovery of head louse prevalence in some communities. Long-term surveillance in Cambridgeshire, United Kingdom, published in 2023, documented precisely this pattern: a pandemic-era decline followed by an unusually gradual return toward pre-pandemic levels. Possible explanations include lingering changes in children&#8217;s behaviour, such as reduced physical closeness, greater spacing in classrooms, altered patterns of group play, and perhaps heightened parental awareness and treatment following the pandemic experience. Broader research on children&#8217;s mental health and socio-emotional development during the pandemic has documented significant disruptions to peer interaction, and it is biologically plausible that these behavioural shifts would be mirrored in the transmission dynamics of a parasite that depends entirely on close contact.</p>
<p>The review also situates head lice within the wider picture of how the pandemic affected contact-transmitted conditions, and here the contrast is instructive. Scabies, another ectoparasitic infestation spread by close contact, showed a more complicated trajectory: some studies reported decreases in scabies and lice cases among paediatric and adult populations during COVID-19, while others documented explosions in scabies cases, particularly where overcrowded households and delayed access to care compounded transmission. Hand, foot, and mouth disease incidence in mainland China fell sharply under pandemic interventions, demonstrating that non-pharmaceutical measures can strongly suppress contact-transmitted infections when they interrupt the right transmission routes. Head lice, by contrast, proved remarkably resilient, because their transmission network is anchored in the household and because the parasite&#8217;s long-lived eggs can sustain an infestation through periods when adult lice would otherwise be vulnerable.</p>
<p>For public health practice, the revised view carries practical implications. First, the pandemic did not eliminate head lice from any community, and clinicians and parents should not assume that a decline in reported cases reflects a decline in true prevalence. Second, the persistence of infestations in isolated households underscores the importance of treating all affected household members simultaneously and of adequate diagnosis, ideally with a detection comb rather than visual inspection alone, since combing is demonstrably more sensitive than simple examination. Third, the slow recovery of prevalence in some Western communities offers a natural experiment in how behavioural change can suppress transmission, but it also cautions against overinterpreting short-term surveillance data. Finally, the divergent outcomes between high-income and low- and middle-income settings highlight the role of socioeconomic factors in pediculosis, a relationship documented in schoolchildren decades before the pandemic. As children&#8217;s social lives continue to normalise, head louse prevalence appears to be climbing back, at its own pace, toward the levels that have made it one of the most common parasitic conditions of childhood worldwide.</p>
