A qualitative study of 21 Turkish women diagnosed with advanced-stage breast cancer identifies limited awareness, fear and denial, and health-system barriers such as appointment difficulties as key causes of diagnostic delay.

In-depth interviews with 21 patients diagnosed at stages 2B, 3, or 4 reveal a tangle of personal, psychological, and health-system obstacles that delayed both diagnosis and treatment. The study, published in BMC Health Services Research, used a descriptive phenomenological design. Researchers conducted individual, semi-structured interviews with patients who met the inclusion criteria and analyzed the transcripts with Colaizzi's seven-step method, supported by MAXQDA 2020 qualitative analysis software.

Because the study is qualitative and limited to 21 participants, its findings describe experiences rather than measure how common each barrier is across the wider population. Six main themes emerged from the analysis: individual awareness of breast cancer, the diagnosis process, post-diagnosis psychosocial status, evaluation of the health system, personal experiences and lifestyle, and treatment experience and its effects. Together, these themes map the journey from the first suspicious symptom to life after a late-stage diagnosis.

On the personal side, limited awareness of breast cancer stood out as a key contributor to delay. Fear and denial also played a powerful role, with participants describing reluctance to confront the possibility of cancer even when symptoms were present. These psychological responses kept some women from seeking medical evaluation until the disease had already progressed.

The health system itself added friction. Patients reported difficulty obtaining appointments and communication problems with providers, both of which stretched the interval between seeking help and receiving a diagnosis. After diagnosis, participants faced significant psychosocial distress.

Yet the interviews also documented resilience. Women described acceptance of their situation, positive reframing of their circumstances, and spiritual coping mechanisms that helped them endure treatment and its effects. The authors conclude that culturally sensitive, nurse-led interventions could address both ends of the problem: reducing early detection barriers such as low awareness and fear, and providing holistic support for the psychosocial burden that follows a late diagnosis.

Because the study captured the accounts of women who had already reached advanced stages, it offers a detailed window into the specific failure points along the diagnostic pathway, though it cannot quantify how frequently each factor operates in the broader population of Turkish patients.
