A national registry analysis of 68,634 stroke diagnoses in Saudi Arabia from 2016 to 2025 finds the burden concentrated in older males, with regional variation and a substantial share of patients overweight or obese.

The study, conducted by researchers from the Saudi Ministry of Health, the Council of Health Insurance and partner institutions using the Population Health Observatory (PHO), is published in Frontiers in Stroke as an observational, registry-based analysis rather than a clinical trial, and its findings describe who is being diagnosed, where, and with what accompanying health conditions. The headline demographic pattern is stark. Nearly two-thirds of recorded stroke cases, 64.45 percent, occurred in males, and the largest share of cases in both sexes fell in the 60 to 69 year age band, which accounted for 23.27 percent of diagnoses.

But because this is a descriptive analysis of recorded diagnoses, the authors are careful to frame these figures as a picture of the registered burden, not necessarily of every stroke occurring in the Kingdom. Geography mattered too. When the researchers calculated the number of recorded stroke cases per 100 people, the highest rate, 0.38 percent, appeared in Najran, a region in the country's southwest.

The authors note that such variation points to the need for a robust national surveillance system capable of tracking where disease concentrates over time. The analysis also examined body mass index among diagnosed patients, finding that 15.28 percent were classified as overweight and 14.47 percent as obese. These figures speak to the role of modifiable risk factors, which the study's authors highlight as a growing concern as Saudi Arabia's population ages and lifestyle-related conditions become more prevalent.

Most patients in the dataset fell into low or rising risk categories, and the majority of healthcare contacts were outpatient visits rather than emergency or inpatient encounters. Methodologically, the study is a descriptive statistical analysis of observational data, and its strengths and limits flow from that design. Its strength is scale: by linking national health data across nearly a decade, it moves beyond the single-hospital and regional studies that have dominated the Saudi evidence base.

Its limits are equally clear. Even so, the authors argue the findings carry direct policy weight. They tie the results to Saudi Vision 2030, the Kingdom's national development programme, which includes ambitious health-system transformation goals.

Concentrated burden among older males, regional hotspots such as Najran, and a substantial share of patients carrying excess weight together argue, in the authors' framing, for strengthened prevention programmes, better management of chronic diseases, and surveillance infrastructure that can feed evidence into national planning. For researchers, the study also fills a documented gap. The authors note that prior Saudi evidence on stroke came mainly from hospital or local datasets, making national estimates scarce.

What this analysis adds is the empirical detail, 68,634 recorded cases, a two-thirds male majority, a peak in the sixties age band, and a measurable regional gradient, that turns a general warning into a targeting map for prevention and care.
