A retrospective conference study found that early consolidative cranial stereotactic radiotherapy was associated with better outcomes among selected patients with EGFR-mutant lung cancer whose brain metastases did not completely respond to osimertinib.

Early cranial stereotactic radiotherapy combined with first-line osimertinib was associated with improved intracranial control and survival outcomes in patients with EGFR-mutant non-small cell lung cancer and limited brain metastases, according to a retrospective study presented at the International Association for the Study of Lung Cancer 2026 World Conference on Lung Cancer. The analysis suggests that the apparent benefit of adding radiotherapy may depend on whether osimertinib produces a complete response within the brain, an association rather than proof that radiotherapy caused the better outcomes.

Researchers reviewed patients with EGFR-mutant non-small cell lung cancer who had brain metastases at baseline and received first-line osimertinib. They compared outcomes among patients who also received early cranial stereotactic radiotherapy with those treated with osimertinib alone. The analysis also examined when radiotherapy was administered and whether patients achieved an intracranial complete response to systemic therapy. Among 255 patients treated with osimertinib alone, 60 patients, or 23.5%, achieved an intracranial complete response.

Patients who did not achieve an intracranial complete response appeared to derive the greatest overall survival benefit from stereotactic radiotherapy, particularly when it was administered in a consolidative approach. In contrast, researchers observed no clear overall survival benefit from radiotherapy among patients who did achieve a complete intracranial response to osimertinib. Compared with osimertinib alone, consolidative stereotactic radiotherapy was associated with improved overall survival, while concurrent stereotactic radiotherapy was not associated with a statistically significant overall survival improvement. These findings support further evaluation of a response-adapted approach in which consolidative cranial radiotherapy is considered for patients with persistent intracranial disease after osimertinib; they do not establish a new clinical standard.

The study reported cumulative incidences of symptomatic radiation necrosis of 9.3% at one year, 24.0% at two years and 28.2% at three years. The retrospective design is an important limitation because differences between treatment groups may have influenced the observed associations, and the analysis cannot determine that adding stereotactic radiotherapy caused the improvements in intracranial control or overall survival. The results therefore provide additional evidence for study rather than definitive guidance for treatment selection. A randomized clinical trial, NCT06020066, is underway to evaluate the clinical value of consolidative stereotactic radiotherapy in this patient population.
