A retrospective study of 327 patients in China found that going home within 48 hours after cryoballoon ablation for atrial fibrillation was not linked to higher arrhythmia recurrence, though total hospitalization costs did not differ significantly.

Patients who left the hospital within 48 hours after cryoballoon ablation for atrial fibrillation fared no worse over one year than those who stayed longer, according to a retrospective real-world study conducted at Fuzhou University Affiliated Provincial Hospital in China. The findings, published in Frontiers in Cardiovascular Medicine, suggest that a structured early discharge pathway may be feasible in a Chinese healthcare setting, where hospitalization practices and peri-procedural management differ from the Western systems where same-day discharge after ablation has increasingly been adopted. The research team drew on the hospital's institutional atrial fibrillation database and included consecutive patients who underwent cryoballoon ablation between January 2021 and December 2023.

Patients were divided into two cohorts according to post-operative length of stay: an early discharge group sent home within 48 hours, comprising 151 patients, and a conventional inpatient group remaining beyond 48 hours, comprising 176 patients. Because patients were not randomly assigned to the two pathways, the investigators used 1:1 propensity score matching to reduce baseline differences between the groups. After matching, 80 patients remained in each group, and baseline comparability improved, although the authors noted that a small residual imbalance persisted for a history of stroke or transient ischemic attack, a limitation that could influence the interpretation of safety findings.

Over 12 months of follow-up, atrial fibrillation recurrence was similar between the two pathways. Recurrence occurred in 8.75 percent of the early discharge group and 10.00 percent of the conventional inpatient group, a difference that was not statistically significant. The study defined outcomes as recurrence of atrial fibrillation, post-procedural complications, and hospitalization cost, providing a combined view of clinical and economic performance for the discharge model.

Safety results were also reassuring for the shorter stay. No major in-hospital complications occurred among patients discharged early, whereas one case of pericardial effusion, an accumulation of fluid around the heart that can complicate ablation procedures, was reported in the conventional inpatient group. Subgroup analyses extended the main finding to patient populations often considered more vulnerable.

Among patients aged 65 years or older, and among those with persistent atrial fibrillation, a more advanced form of the arrhythmia in which the irregular rhythm is sustained rather than intermittent, recurrence rates showed no significant difference between the early discharge and conventional inpatient groups. This suggests that age and arrhythmia type did not clearly modify the effectiveness of the shorter hospitalization model in this cohort. The economic picture was more nuanced.

The early discharge group incurred significantly lower medical service costs and significantly lower medication costs than the conventional inpatient group. However, no significant difference emerged in total hospitalization costs between the two groups. The study's context matters for interpreting its conclusions.

Same-day discharge after atrial fibrillation ablation has been increasingly adopted in Western healthcare systems, but its applicability in China remained uncertain because of differences in healthcare delivery, peri-procedural management, and hospitalization practices. Real-world evidence on early discharge pathways for ablation in the Chinese setting had been limited, and this analysis from a single center's database offers one of the first assessments of clinical and economic outcomes for cryoballoon ablation patients managed under a shorter stay model in the country. The authors concluded that, in this real-world Chinese cohort, a structured early discharge pathway for cryoballoon ablation appeared feasible and was not associated with a higher atrial fibrillation recurrence rate, while reducing medical service-related costs.

They called for larger prospective studies to further confirm the safety, effectiveness, and economic implications of the model before it is more widely implemented.
