										<content:encoded><![CDATA[<p>A large randomized trial of older adults has delivered a sobering message for the rapidly expanding mindfulness industry: an eight-week mindfulness-based stress reduction program did not improve fluid cognition or driving-related cognitive abilities more than a structured lifestyle education course. Yet the study also found that participants in both programs performed better on cognitive tests over the following year, raising a more complicated possibility—that regular participation in intellectually engaging, socially connected group activities may support cognitive performance even when mindfulness itself offers no special advantage.</p>
<p>The findings come from the Healthy Agers randomized controlled trial, conducted by researchers at The Ohio State University and collaborating institutions. The secondary analysis included 150 adults between 65 and 85 years old, with 76 assigned to mindfulness-based stress reduction, or MBSR, and 74 assigned to an active comparison program called Lifestyle Education, or LifeEd. Most participants were cognitively healthy, although 18 met criteria for mild cognitive impairment. The researchers assessed cognition before the intervention, immediately afterward, and again six and 12 months later. Their central question was whether training attention and awareness through meditation could produce measurable gains in mental abilities relevant to daily independence.</p>
<p>Fluid cognition refers to the capacity to reason through unfamiliar problems, process new information, inhibit inappropriate responses, remember information temporarily, and shift flexibly between mental tasks. Unlike crystallized intelligence, which relies heavily on accumulated knowledge and experience, fluid cognition is often considered more vulnerable to aging. The study measured it using the National Institutes of Health Toolbox Cognition Battery, a standardized collection of computerized and performance-based tests. Scores were converted into demographically corrected T-scores, taking account of factors including age, education, sex, and race or ethnicity. Higher scores represented stronger performance.</p>
<p>The study also examined driving-related cognition using the Driving Scenes subtest of the Neuropsychological Assessment Battery. This task requires participants to interpret visual scenes and identify information relevant to driving, including hazards, traffic conditions, and spatial relationships. It is not the same as placing someone behind the wheel, but earlier research has found a modest relationship between performance on the test and real-world on-road driving. That distinction matters: a person can improve on a laboratory measure without becoming a safer driver, and a cognitive test cannot capture every factor involved in driving, such as judgment, physical reaction, vision, confidence, and road experience.</p>
<p>Both interventions were deliberately substantial, making this a demanding test of whether mindfulness provides benefits beyond participation in a well-organized program. MBSR followed the model developed by Jon Kabat-Zinn and included weekly group sessions lasting approximately two and a half hours for eight weeks, a longer retreat, and four booster sessions during the year-long follow-up. Participants were asked to practice at home for 30 minutes on five days each week. The LifeEd group met for the same general schedule and received evidence-based lessons on health topics, alongside stretching and toning exercises. Participants in both groups used a web and mobile application to record their home practice, while facilitators tracked attendance.</p>
<p>The statistical analysis used random-intercept linear mixed models, a method designed to account for repeated measurements from the same individuals over time. The researchers also used multiple imputation to address missing data, which was substantial because only 85 participants completed the 12-month assessment. Rather than simply comparing the people who remained in the study, the intention-to-treat approach aimed to preserve the original randomized groups. The main test was the group-by-time interaction: if MBSR had a distinctive cognitive effect, the trajectory of the meditation group should have differed significantly from that of the lifestyle education group.</p>
<p>It did not. For both fluid cognition and driving-related cognition, there was no statistically significant group-by-time interaction. The pre-specified comparison of changes from baseline to the immediate post-intervention assessment also found no meaningful advantage for MBSR. The estimated difference between groups was approximately half a T-score point for fluid cognition and 1.63 points for driving-related cognition, neither of which was statistically significant. The same pattern held when baseline scores were compared with the average of the six- and 12-month follow-up estimates. There was also no overall main effect of group, indicating that one intervention did not consistently produce higher cognitive scores than the other.</p>
<p>The more encouraging result was a general improvement across time. Fluid cognition scores increased from baseline to the end of the intervention and rose further at six and 12 months. Driving-related cognition also improved significantly at six and 12 months, although its immediate post-intervention increase was not statistically reliable. These gains occurred in both groups. The researchers therefore concluded that older adults can show improved performance after sustained involvement in structured programs, but the study cannot establish that meditation caused those improvements. Repeated exposure to the same or similar tests can create practice effects, as participants become familiar with the task format, strategies, and timing. Because the trial did not include a waitlist or no-contact control group, it cannot separate genuine cognitive change from improved test-taking skill.</p>
<p>The researchers also investigated whether the amount of mindfulness practice predicted cognitive improvement among participants assigned to MBSR. Baseline cognitive performance was, unsurprisingly, the strongest predictor of post-intervention performance. Attendance and logged homework practice did not make statistically significant contributions for either outcome. Attendance was particularly difficult to use as a discriminating measure because adherence was high: participants attended an average of 7.85 of the nine major sessions, and 80 percent attended at least eight. When most people receive a similar dose, there is relatively little variation available to reveal a dose-response relationship. The absence of an association therefore does not prove that practice is irrelevant, but it does mean that this study found no evidence that more meditation translated into better fluid or driving-related cognition.</p>
<p>The result challenges simple claims that mindfulness is a universal cognitive enhancer, while leaving room for more targeted effects. Earlier research has reported small benefits of mindfulness in older adults, particularly in attention, long-term memory, and visuospatial processing, but rigorous trials have often produced mixed findings. The primary Healthy Agers analysis found modest reductions in self-reported task-unrelated thinking, suggesting that MBSR may influence mind-wandering without necessarily improving broad neuropsychological performance. In the comparison group, social interaction, facilitator contact, health education, and physical activity may have provided their own cognitive stimulation. Stretching and toning are not equivalent to aerobic exercise, but movement, social engagement, and regular attendance can all affect motivation, mood, arousal, and test performance.</p>
<p>The study’s population also limits how widely the findings can be applied. Participants were generally healthy, highly educated, technologically comfortable, and able to perform light physical activity; nearly three quarters were White. Such a sample may have had limited room for improvement, particularly if many participants began near the upper end of the tests’ useful range. The intervention was also moved from in-person delivery to live online sessions during the COVID-19 pandemic, although sensitivity analyses did not identify clear differences associated with delivery format. Attrition was uneven at 12 months, with 35 of 76 MBSR participants and 50 of 74 LifeEd participants completing the final assessment, increasing reliance on imputed data for the long-term results. The researchers further note that the sample was powered to detect medium-sized effects and may have missed smaller benefits.</p>
<p>For now, the Healthy Agers findings support a cautious interpretation: mindfulness-based stress reduction may be valuable for well-being, stress management, or attentional habits, but this trial provides no evidence that it uniquely improves fluid reasoning or driving-related cognition in older adults. The cognitive gains observed across both programs may reflect practice with repeated testing, the benefits of social and intellectual engagement, the effects of physical activity, or a combination of these factors. Future studies will need no-contact or waitlist groups, larger and more diverse samples, objective measures of everyday function, and ideally real-world driving outcomes rather than laboratory proxies alone. The most viral version of the mindfulness story may promise a sharper mind through meditation; the more scientifically defensible story is subtler. For older adults, doing something structured, challenging, and socially connected may help—but this trial found no special cognitive shortcut in mindfulness.</p>
