A cell-model study identifies clusterin as a downregulated gene in sepsis-associated acute lung injury and shows that CLU overexpression limits apoptosis by interacting with Bax and blocking mitochondrial damage.

Sepsis-induced acute lung injury remains one of the most lethal complications of severe infection, marked by runaway inflammation and widespread death of lung cells, yet the molecular pathways driving this damage are still only partly mapped. A new study published in Molecular Biology Reports on 22 October 2026 points to clusterin, a protein encoded by the CLU gene, as a potential protective player. Using a combination of bioinformatic screening and laboratory experiments, researchers identified CLU among a small set of genes whose activity drops in sepsis-associated lung injury, and went on to show that restoring CLU in a cell model of the condition limits apoptosis and preserves mitochondrial function.

The team began by mining the GSE10474 dataset to find genes differentially expressed in sepsis-induced acute lung injury. A protein-protein interaction network analysis then narrowed the field to six hub genes that were downregulated in the condition: CDKN1A, HIST1H3F, HLA-DQB1, CD74, VDAC1 and CLU. The researchers validated the reduced expression of CLU experimentally, measuring it with quantitative reverse transcription PCR and Western blotting in an in vitro model of sepsis-caused lung injury. This convergence of computational prediction and laboratory measurement singled out CLU as a candidate worth probing further.

When the researchers overexpressed CLU in the injured cell model, the effects were striking. Cell viability recovered, and apoptosis, the programmed form of cell death that spreads through injured lung tissue, declined. Because mitochondrial failure is a central trigger of apoptosis, the team then examined the organelles directly. Transmission electron microscopy revealed that CLU overexpression helped maintain normal mitochondrial morphology. Consistent with that, the cells retained higher ATP content and preserved their mitochondrial membrane potential, the electrical gradient that mitochondria need to produce energy, while the accumulation of reactive oxygen species, a hallmark of mitochondrial stress, was reduced.

The mechanism appears to run through Bax, a well-known pro-apoptotic protein. In apoptotic signaling, Bax translocates to the mitochondria, where it promotes the release of cytochrome c, a step that commits the cell to death. Co-immunoprecipitation experiments showed that CLU physically interacts with Bax and inhibits its movement to the mitochondria. By keeping Bax away from the organelle, CLU curtailed cytochrome c release, which in turn helped preserve ATP levels, membrane potential and mitochondrial structure while limiting reactive oxygen species buildup.

To test whether Bax was truly the decisive target, the researchers overexpressed Bax in cells that already carried extra CLU. The protective effect of CLU was attenuated: apoptosis increased, cell viability fell, reactive oxygen species rose, and mitochondrial damage worsened. This loss-of-protection experiment strengthens the case that CLU acts at least partly by restraining Bax-dependent mitochondrial damage, rather than through some unrelated pathway.

The authors conclude that their in vitro findings suggest CLU could mitigate apoptosis in the sepsis-induced acute lung injury cell model, at least partly through modulating Bax-dependent mitochondrial damage, and they propose CLU as a possible treatment target for the condition. The scope of the evidence matters, however. All functional results come from a laboratory cell model of the injury, not from animal models or patients, and the abstract itself frames the mechanism with the qualification that Bax inhibition may contribute to the observed effects. Translating these findings into therapies for people with sepsis would require the kinds of validation studies that lie beyond this work. Still, by connecting a downregulated hub gene to a concrete mitochondrial death pathway, the study offers a defined molecular thread that future research on sepsis-associated lung injury can follow.
